1393017683 2014-02-21 I called Jordan Jones of G-R Contracting at 11:15 am. I asked him when he was going to move a dozer and water truck to the site to kill the flow of sour gas. He stated that the county roads were too soft and he did not want to tear them up with the heavy trucks. I agreed to allow them to wait until next Tuesday for the road conditions to improve.

169267478 2014-01-31 The annular disposal is not in use. The well is equipped with 4.5 casing and 1.5 tubing. All the valves are shut. The well has a significant lean. It appears the casing has failed at the surface. There is a strong odor of sulfur at the well. I will return on Monday to test the area for H2S. The lease road is impassible.
-2121904182 2014-03-27 My inspection finds the well has been returned to production. The previous violations have been corrected. The flow of H2S has been stopped. The well is equipped with 1.5 tubing on a packer and is being produced through an intermittent timer. A meter is set on location and it has a current chart.
523912195 2014-02-06 My inspection finds the well is equipped to be produced by a plunger lift system. All valves are shut in at the well. A test of the area around the 8.625 casing, with a Honeywell PhD6 gas meter, indicated that H2S gas was present in amounts of 80 ppm. I contacted Mr. Shulzberger with G-R Contracting and made him aware of the issue. Mr. Shulzberger was directed to immediately cordon off the area around the well and post signage notifying people of the danger. I also directed Mr. Shulzberger to submit a plan in writing that outlines the company plan to kill the well to prevent the flow of H2S. Additionally I asked for a plan to remedy the well long term with a specific time frame. IMMEDIATELY STOP THE FLOW OF H2S. IMMEDIATELY REPAIR THE CASING, CORRECT THE CONSTRUCTION INADEQUACIES, OR PLUG AND ABANDON THE WELL. SUBMIT, IN WRITING, A PLAN WITH TIME LINES TO ACHIEVE THESE REQUIREMENTS WITHIN 72 HOURS.
-81323815 2014-02-25 The company sent a dozer to repair the lease road. Once accessing the location we determined that the 4.5 casing has no pressure and the tubing has over 600 psi. The pressures indicate the tubing may be set on a packer. The 8.625 casing has over 150 psi and the gas has traces of H2S. I instructed the company representative to flare the gas off of the 8.625 casing and load the hole with fluid to stop the flow of gas.
942950055 2014-02-05 I called Jordan with G-R Contracting at 1:06 pm. I spoke to him about the condition of the well. He stated that a truck had struck the well sometime ago and caused the well to lean. I informed him that I detected a strong odor, that may be H2S, on my 1/31/14 inspection and that I would return to test the well as soon as the weather permitted. I also told him that if the well was leaking H2S the company would be required to take immediate action.
