1337194263 2012-05-16 My inspection found pollution at the well head and natural gas bubbling below the 4.5 inch casing. I called the number listed for G & O Resources Ltd (330-761-3508) and it went to a fax line. I then called the emergency number listed on the well identification and spoke with Dave Widuck (330-323-6732) and he advised me he would have it cleaned up with in the week. The record shows the well has reported production for 2010 and no other violations are noted at this time.
1338480945 2012-05-31 My inspection found the well no longer leaking and the requirements of the Compliance Notice have been met.
1429727313 2015-03-31 I conducted a status check of this well with DOGRM inspector Heidi Scott. Our inspection found this well to be equipped with an electrically-driven pumpjack. The polish rod was not lubricated. Fresh oil measuring 88 inches by 77 inches surrounded the wellhead, and there was active bubbling through the oil at time of our inspection. The tubing valve coming out of the wellhead was open. We were unable to determine if the production casing valves were open at the time of our inspection because they were coated with fresh oil. Inspection of the tank battery found that it consisted of one-210 bbl production tank, one-210 bbl tank labeled as a brine tank, horizontal separator, a meter run, and a heater treater separator. There was also an unused 100 bbl tank with no valves, and an unused 100 bbl tank that was located outside of the dike. This well shares a tank battery with G &O wells PN # (364, 365, and 383). This pad is shared with Petrox tank battery PN # (1102 and 1103). There was an oil stain around the separator measured at eleven inches by ten inches. The brine line at the back of the 210 bbl tank was leaking brine, and had formed a puddle measured at 32 inches by 17 inches. At the time of inspection, the brine was dripping at a rate of 2 drips per second, and there were salt crystals present under the brine line. There was four inches of standing water adjacent to the heater treater inside of the dike with two inches of oil on top of the water. The dike was heavily eroded and there was evidence that it had been previously breached. Weathered oil was present outside of the dike, and oil stained soil was present on the haul road.
