1315512967 2011-09-08 My inspection of the well site showed final grading has been completed and vegetative cover is established with no erosion noted. There is a stack of 8 and 4.5 inch casing on site and frac tanks still located along lease road entrance. There is no identification posted at the well or tank battery. The owner must remove the casing and frac tanks from the site and any areas where soil has been disturbed due to removal of this equipment shall be graded, seeded, and/or mulched. Also, proper identification must be posted at the well and tank battery.
1318856529 2011-10-17 Conducted a Notice of Violation follow-up. Ther casing has been removed from site. Proper identification is still not posted by either the tank battery or the well. The frac tank is still on-site, down by road. I spoke with Mike McPeak about these issues.
943823958 2010-11-15 Called received for further frac. Arrived this day as frac trucks leaving. Well completed in Berea with 7' perf'd, well broke down at 1200#, treating at 400# and 3 bbl/min. Well is remain shut in until tomorrow when rods, tubing will be run and jack set. Well will then be pumped into stock tanks. 1000 bbl water used.
