Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1608643246 2014-12-08 I inspected this well on 12/8/2014 as a follow up to a previously issued Notice of Violation (# 747094967 issued 11/2010 ). The corrective measures prescribed previously have not been completed and some new violations were identified. The production has never been report to RBDMS. The required I D signage is illegible and incomplete. A locked fenced surrounds the tank battery. The tank battery is 80' from the pitchers mound of the ballfield. The tank battery consists of a 100 barrel oil tank, an in ground concrete brine vault and horizontal separator. The heat tube on the 100 barrel oil tank is leaking and causing ground contamination. The tank will have to be replaced or fixed. The wellhead is fenced and locked. The well head is 20' away from the soccer field. The well is comprised of 4.5 casing and is a swab well. In order to bring the Copley - Rohrer #1 into compliance the Operator shall: 1.) Comply with the violations as detailed in Notice of Violation # 747094967 ( statement of annual production not filed ), 2.) install new I D signage ( urban requirement ), 3.) repair or replace the leaking 100 barrel oil tank. The old and new violations shall be completed and approved ( by DOGRM Inspector ) by 1/30/2015
161761855 2011-01-10 The required identification has been posted but the natural gas sales line connecting the #2 well with the #1 well still has not been replaced. It is my understanding this will occur when weather permits. The annual production violation will be corrected when production is reported for 2010.
-747094967 2010-11-15 I received a call from Paul with Dominion East Ohio (330-351-1061) and Steve Robinson with the Copley Fairlawn School system (330-664-4810) reporting a natural gas odor in front of the High School. Dominion reported that the leak was not from one of their pipelines. There are two wells on school property that are owned and operated by Karl Rohrer (330-801-0043). The sales line connecting the #2 well (PN2335) with the #1 well (PN 2135) does cross in front of the High School. I spoke with Mr. Rohrer who informed me that Steve Hilliard pumps these wells. I advised Mr. Rohrer and contacted Steve with the order to shut the #2 well in until the pipeline is replaced. I also ordered that the required identification be placed on the respective tank batteries for each well. Annual production has never been filed for this well and must be corrected.
