1460173683 2012-10-17 I went to the site to check the status of the cellar floor. Jimmy Freeman of Chesapeake indicated that it would be completed with redi-mix the next day.

-1887554950 2012-10-15 I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 120'. Express installed 120' of 20 OD casing. During drilling, water was encountered at an approximate depth of between 55 and 60'. Allied Cementing Co., LLC was the cementing contractor. The cement was mudscaled at 16.2 pounds per gallon (ppg), and adjusted to 15.6 ppg. Allied pumped 47 bbls of cement slurry at 15.6 ppg. The cement was displaced with 40 bbls of water. Cement returns to surface were observed. An estimated 10 bbls of cement returned to surface. Displacement was completed at 8:47pm.
-1889816396 2013-07-02 I reviewed the site to assess the completion of preliminary restoration. There is a sign board at the access road entry featuring the well name, permit number and emergency contact information. The access road has a padlocked gate near Winona Road. The access road was in good repair, and drainage and erosion controls were in place and appear to be functioning. Ground cover was well established along the access road. I found the drilling/production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. The pad has a crushed limestone surface, and is in good repair. Along the interior of the chain link fence there are two site drains installed
312237210 2012-10-15 I was on site for the cementing of the conductor for this well location with Rick Anderson. I witnessed cement come back to surface. The pad is in good condition and there are no violations at this time.

A number of frac tanks were present on site, and a water pumping set-up was present on Depot Road where it intersects Cold Run Creek. A crew was installing a security camera. No one else was present.
