-1221050576 2013-10-07 Met with Hal Miller and Rick Triple on site to review the clean up of buried cutting which are being removed. The cuttings have been removed to a landfill and now the area which was below that area needs to be tested for chlorides. If the area tests are near normal then the entire area can be back filled and graded.
-1258686648 2014-01-21 On the day of my inspection the site passed preliminary restoration according to 1509.072A.
-1521875306 2013-09-18 I am on site today to check on the clean up of cuttings which are buried on site. I found that the well has a coil tubing unit working on the well with a wire line truck. I met Hal Miller of Gulfport on site to go over the clean up of the cuttings. Hull and Associates are on site as well to collect samples to make sure all of the cuttings have been removed. They are about 70% complete with the removal of the cuttings.
-422109928 2013-08-21 I am on site today to check on the repairs to the containment liner. I found that all of the fluid running under the liner has not stopped but increased. I collected quantab samples and found chloride in 450 to 500 PPM range. I contacted Hal Miller of Gulfport Energy and he is in route to my location. I also contacted Deputy Chief Bob Worstall and he said for me to have Kyle Tharp help with samples. Bob is concerned that the well site is on Mine fill and could be leaking out and under the drilling pad. During my inspection I found that the soil samples which were to have been removed are still on site. The cuttings were buried on site two years ago after the drilling was complete on first well. The Division was not notified that the cuttings were on site and I was informed that they would be removed the same day I was notified. Hul and Associates to send Oil & Gas a copy of the maps, and all material results. Kyle Tharp collected samples and will transport them to the Cambridge lab for testing. 9-1-07
-709254200 2013-06-20 I am on site today to witness the running and the cementing of the 16" inch diameter casing string. Patterson Drilling Rig # 56 has drilled a 20" inch diameter hole to 315 ft and set 305 ft of casing. Universal Cementing Service c/w 500 sks of class A. Could not get casing to bottom and had to be pulled out and the wellbore conditioned. Returned the next morning after casing was ready to cement and did get 25 bbls of good cement returns.
1316927101 2013-08-29 I am on site today meeting with Hal Miller. Hal wanted to meet to go over the containment changes which they would like to make. At this time the containment is leaking in several places and needs pulled up. They want to move the tanks to flow back into to an area where the containment is good and complete the work on the wells. I advised that the tanks could be moved from the area where we know the containment needs repaired to another good area. After the work on the wells is complete in two days all of the containment is to be removed. I also advised that if the poor containment area was pulled up when I could not witness and document that somebody would need to collect photos and provide ODNR with those photos. Hal Miller advised that he would collect photos if I could no be on site to witness.
1375967765 2013-08-08 I am on site today because I received a call from Gulfport to advise me that during the installation of the production line they found oil base mud around the tin horn. I am on site today to make sure all of the oil base is removed. I advised them to dig a trench then flush the area with freshwater and vacuum up any residue.
1523229284 2013-10-22 I am on site today to check on the operation status of the well and to review the clean up of the spill which occurred 08/21/2013. I found that the well is in production and that the clean up of the site has been complete. The site also had some cutting buried on site which I had advised the operator to move. Those cuttings have been removed the area has been graded and seeded along with mulch for cover. The violation has been corrected.
188138094 2013-08-22 I am on site today and met with Hal Miller to review the repairs to the containment pad and to check on a further leaks. I found that the containment is partly repaired and it has a vacuum line to remove the fluid before it runs over the containment berm. There is still fluid leaking out from under the containment pad which is also being contained with the vac. The old cuttings on site are going to be removed after the work over portion of the well is complete.
245910050 2013-10-22 On the day of my inspection I witnessed that the clean-up was completed of the contaminated area and per request seed and straw were in place for erosion control.
714257823 2013-09-16 There is a service rig over the well today running tubing and pulling tubing. The containment appears to be in place and holding at this time. The cuttings which are buried on site are being removed today. The cuttings which are from the # 1 well are 50% removed at this time. Hull Associates on site to test to make sure all of the cuttings are removed. I will return next day to verify as well.
788920601 2013-08-26 I arrived today to check on the status of the leak coming out from under the containment and off the end of the containment. The leak area which was sampled has reduced some. The leak from off the end of the containment is on going. It has red dye in it and I tested it with for chlorides and found it at 160 PPM or below. I advised Hal Miller that the containment was still leaking and that there was red dye running down the road. The operation on this well is almost complete and we will be able soon to pull back the containment and see where the leaks began.
