-1044367607 2013-11-06 This is an annular disposal inspection as part of a request from Columbus DOGRM. There is one 100bbl tank with a knockout and a separator in the tank battery. The annular disposal appears to be hooked up with an open valve, however there is another disconnected pipe sticking up on the backside of the wellhead which could be the actual line. There is no motor on the pumpjack. The dike does have contamination and several small line leaks. There is standing water on one side of the dike which Quantab (low range) tested >700ppm of chlorides. I called the operator and he stated the well tender had disconnected the annular disposal and are in the process of getting the well back into commercial production. Their motor was recently stolen and they just bought a new one. They will repair any leaks and cleanup any contamination. The operator stated the well tender Dave Paul will call me to discuss the annular disposal work. 11/8/2013: Tim Altier stated the AD was only disconnected at the tank battery. They called Columbus and plan on performing a mechanical integrity test on wellhead in order to maintain annular disposal opportunity. If this does not work for them then they will file for a plug permit. NONEI
-2053416436 2014-03-25 Compliance Action Follow-up -1044367607: The well is currently shut in and there is an old chart in the meter box. There is no motor on the pumpjack and there is still contamination in the dike. I talked with the bookkeeper, Ernie, and stated they plan on plugging the well and will be sending in the plug permit this week. I told him about the dike issues and he said he would relay the information to Benny Altier. Benny called back and stated he would take care of the issues.

1077401155 2014-08-21 I was on location to witness Formation set the surface casing plug using 55 sacks of Class A cement. Fluid level was around 275' so they set a brush and stone plug. Formation then set a 200' surface plug using 47 sacks of cement. The cement stayed at the surface.
1214651737 2008-06-27 On site to witness MIT Test,Ohio Pump of Logan preformed test, worker pressured up on annulus to 445 psi with Nitrogen and we monitored pressure for 1 hr. no decline in pressure test passed,called company rep Jeff Marshall and requested cleanup around wellhead before placing well back into production he stated he would call after cleanup was done so I could inspect site for compliance.

There is no motor on the pumpjack. The dike is covered with ice and snow so I will come back after a change in the weather to follow up on the contamination issues.
