-1005487630 | 2012-09-04 | Arrived on location with Josh Garland and Jake Glascock. JD Drilling's Service rig and water truck were on location. They were rigged up waiting on a tool to lift the production tubing packer, R4, to reseat it before the mechanical integrity test was performed. According to Josh they have already tried once on an earlier date to conduct this test and it did not pass. Annular space was already loaded with fresh water on top of the packer before we arrived. Josh put his 1000 psi gauge on the annular space and they used air pressure to raise pressure on the water column to 505 psi to start the 15-minute integrity test. At the end of the test, the well pressure measured 481 psi and met compliance by 1.25 psi ( 505 X .95 = 479.75 psi & 481 - 479.75= 1.25 psi). Josh then measured the injection pressure at 0 psi.

-1454907603 | 2014-06-17 | Lease road has been graded and gravel has been applied to 3/4 of the road. The lease road is still in use for production wells on the lease. The concrete containment dike, one 100 barrel steel tank, injection pump, lines, and electric service has not been removed. I called Spence Carpenter and Spence told me that they are not sure if they are going to remove the containment dike or reuse the dike and tank for production on the lease. I told Spence that He needs to remove the dike and equipment or submit a waiver form to the UIC section.
-1595126566 | 2014-01-02 | Meeting at JD Drilling office with Spence Carpenter (JD Drilling), and Inspector Josh Garland. We discussed the plans for getting the well in compliance. Spence told us that the plan is to rebuild the R4 packer and install it with a string of new tubing some time next week between 6th and 10th of January 2014. Weather permitting.
-16800175 | 2004-02-05 | Pressures were measured, while the pump was off.
-1683044888 | 2004-07-20 | The previous violation of tubing/fitting requirements has been remedied. A new valve and gauge have been installed on the tubing. Unable to remove pressure gauge, on the tubing, with an adjustable wrench. This gauge is showing a pressure of 0 psi., however this may actually be a vacuum.
-1687047299 | 2007-02-16 | Liner will need replaced.
-1770751467 | 2012-05-07 | Annulus is on vacuum. Filled annulus with 15 barrels fresh water and inhibitor, and attempted to test. Unable to pressure up on annulus. Spence Carpenter said it could be up to two months before they could get a rig on the well to pull the tubing and packer. Injection is shut down.
-17872069 | 2013-07-23 | Follow up to -2075986878. Roughly 30 gallons of fresh water used to fill the annular space. Further remedial actions will be taken and well will remain shut in untill mechanical integrity is proven.
-1879729485 | 2013-06-06 | No violations.
-1897236552 | 2005-06-16 | Upon arrival, the pump was not operating and there was no pressure on the tubing.
Attempted to increase the annular pressure. However, the pressure would not exceed 80 psi.. Instructed the well owner's representative (Spencer Carpenter) to shut down the operation and requested a suspension order. |
||||
-2075986878 | 2013-07-17 | The injection tubing was on vacuum. Gauges read no pressure on the annular space, but after opening the needle valve it was noticed the annular space was full of pressurized gas, not fluid. Pressure differential exists between injection tubing and annular space, mechanical integrtiy is still intact. However annular space should be full of fluid. | 9-3-07(H)
