-1382260031 2013-01-09 FOLLOW UP TO NOV # 1357910173 Well in operation at the time of inspection. There is a containment dike around the well head and there is 2-3 inches of fluid in it. Production has not been reported since 2009. Identification is posted on the pumping unit and the tank. The fluid around the well head was discussed with Mr. Albert Poling on 1/10/13 at 9:28 am via telephone. He keeps regular checks on it and will continue to keep it pumped out. We also discussed the issue of not reporting production and I gave him the number to call and get a hold of Patty Nicklaus at the Columbus office so that he can acquire the forms needed to do so. There is one 100 barrel tank that the well pumps in to. There are no compliance issues there. Production records need to be submitted for this well.
1267684752 2010-02-02 An inspection revealed 50' x 15' path of old oil (paraffin) over a hillside from the well location. Following the path of P&C, the source derives from the stuffing box. An earthen dike will be required at the well head. No identification observed at the well or tank. Currently, there are no statements of production for this well in RBDMS. The well consists of a pumping unit with production equipment at the well head. New motor with no belts observed on unit. A 3" hose connected from casing side of well routed to a separator containing 3/4" lines (2). A 3/4" line was also attached to the pumping tee side of well. A 6'x 4'x 3' steel box located behind the well with a 3/4" line attached at bottom then routed to top of lid. A 100 bbl tank was also located on the property. 2" steel was observed as the connection into top of tank. Unable to obtain GPS reading for well and tank, due to weak signal. Remove all contaminated material at the well location. Construct an earthen dike around the well head. Identification for the well and/or storage tank is required.
