1239192269 2009-04-02 Email received in office on 04-02-09from landowner near the well drilling wanting to know what the noise is all about. Called company talked with Ben Fouderburg he advised me that the drilling should be completed on Thursday or Friday at the latest. Called Mr. Herbst and advised him of what is going on near him and that the State regulation supersede the city ordinance on noise at night.

1239721006 2009-04-02 The drilling will be done by 6PM tonight. The company will be logging and cementing 4.5" in morning. Phone call to Mr. Herbst to inform him of the information on the noise complaint.

264158586 2014-07-16 I inspected this producing unit as part of routine inspection activity. The well was completed in 09/28/2010. The unti is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and pump jack system. The wellhead is located in a closed and locked chain-link enclosure. There is oil leaking from the stuffing box and staining the interior ballast area around the wellhead. The aboveground storage tank battery is located south of the well, along the access road. The battery is comprised of a total of 3 steel 140-barrel production tanks, 50-barrel poly brine tank, and horizontal separator. Spacing from well to battery, and spacing of elements within the battery meet regulations. The battery is located in a separate chain-link enclosure which is gated and locked. The load-outs are bull-plugged and locked. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. I contacted Bob Allen with Ohio Valley regarding the oil leak at the stuffing box.
