-1243919773 2012-03-13 FOLLOW UP TO NOV # -93452657 I received a call from Phil Sharp (330-938-0441) on the evening of March 12, 2012 requesting an inspection of a well on his property that he reported as being incapable of production. On the day of my inspection I conducted a status check at the site and found that the well was equipped with a pumping unit and there was evidence of the well being recently serviced. The soil was disturbed around the well head and excavating equipment remained at the site. The well head was bubbling gas in two places and there were pools of oil/brine contamination around the casing. The tank battery consisted of one 210 barrel stock tank, one 100 barrel brine storage tank, one separator and a dike. Proper identification was posted on the tank battery and the dike was overgrown with weedy vegetation. The chart was dated 07-16-2009, and the ODNR database revealed that no record of production has been filed since 2009. I tried to reach Dart Oil (303-355-8341) by phone on March 15, 2012, with no success.
-147680523 2012-12-06 I performed a fellow up inspection on the above location the tank battery consist of 1-210 barrel tank, 1- 100 barrel tank and a horizontal separator. The dike has been rebuilt and is in compliance. The well is equipped with a pumping unit and a electric motor. All violation have been cleared at this time

We discussed the violations and the actions that need to be taken to bring this well into compliance. On the day of my inspection I found that the soil had been graded and a dike had been constructed around the pumping unit. A new electric box had also been installed. The pumping unit showed no signs of actually pumping and the well head was still leaking. The tank battery was still overgrown with weeds and a new chart had been installed, which showed no recorded production. This NOV continues. NONE_
