-1669089934 2013-05-17 On the day of my inspection Cemenco hooked up to the 4.5" casing to circulate through perforations. The pressure increased rapidly when attempting to circulate. The pressure was not allowed to increase over 1200 psi. Cemenco shut down without pumping any cement. The casing will need to be perforated again.

Call from Bill Arnolt @ 4:30 pm w/ Northwood Energy 3/12/09 . 3/13/09 inspection of oil spill at well site . 210 bl. Storage tank inside dike , the dike contained all but 0.5 bls. Oil ran into drain tile at well site and came out down hill approximately 100' covering a area of 40' by 70' . Oil and contaminated soil have been removed from this area oil spill clean up is on going at this time . Storage tank is 25' from well head . Storage tank is in violation of spacing . Tank will need to at a minimum of 50' from well head & diked to contain 110 % of storage tank . No identification at well or tank . 1368799957 2013-05-16 Appalachian Well Surveys set a cast iron bridge plug at 5500' and a drillable bridge plug at 5266' Then they perforated the 4.5" casing with 6 shots at 5262'.

1993741583 2013-11-12 On the day of my inspection I found that the well site was graded out to the approximate contour of the surrounding landscape and covered in well established short grasses. This plug back site passes preliminary restoration.
