1150617999 | 2018-03-13 | Preliminary Restoration | No Violations | On the day of my inspection I found a Universal frac crew setting up the pad for frac job. Water tanks and containment are onsite, no frac pump. 7-15 frac stacks are on all wells. 2H, 5H, and 7H are the first wells planned for stimulation. Outslopes of pad are vegetated and stable. Site meets requirements of ORC 1509.072A for preliminary restoration. |
1445161022 | 2018-06-12 | Preliminary Restoration | No Violations | On the day of my inspection I witnessed that the pad has been brought online. Pad has 5 properly labeled wellheads. Production equipment includes two 500 barrel steel tanks, five GPUs, two line heaters (for the 5H and 7H wells), and seven sand seperators. These wells do not have tubing in them and are produced through the 5.5 inch diameter production casing. Set up appears to be a frac stack with well head on top of it. Flowback operations onsite have a target goal of 18.2 mmcf/day for this well. Surface annular pressure is 50 psi, intermediate annular pressure is 851, and production pressure is 4715 psi. Flowback is finished on this well. Pad is stable and vegetated. Site meets requirements of 1509.072A for preliminary restoration. |
1508330813 | 2017-10-18 | Casing String Witnessed | No Violations | On the day of my inspection I witnessed Precision Rathole had augured a 36 inch diameter hole to 80 feet. 80 feet of 30 inch diameter conductor casing was run. O-tex was onsite with 280 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% percent excess ran. Cement was displaced with 54 barrels of fresh water. Cement returned to surface and then fell back 2 feet immediately. Checked it 30 minutes later and was cement was at same level. They had to put a nipple and cap on the valve as it was leaking. The remaining two feet will be filled in with cement when the floors are filled in. |
1508863096 | 2017-10-24 | Casing String Witnessed | No Violations | I am on site today to witness the cementing of the 18.625 inch mine string casing. Patterson Drilling rig 284 drilled a 24 inch diameter hole to a depth of 225 feet and set 200 feet of 18.625 inch, 87.5 pound per foot casing. Schlumberger pumped 25 barrels of gel then 146 sacks of class A cement, 1.2 yield, 15.6 pound per gallon, surface set. Two coal seams were identified from 110 feet to 114 feet and from 136 feet to 144 feet, two baskets were set at 105 feet. Schlumberger displaced with 50 barrels of fresh water to land the plug. Cement did not returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 630 psi for 5 minutes. Patterson ran one inch tremie pipe down the backside of the 18.625 inch casing. Schlumberger pumped 4.3 barrels (20 sacks) of class A, 1.2 yield, 15.6 pound per gallon cement to set the basket. After waiting approximately 2 hours Schlumberger pumped an additional 37 barrels (173 sacks). After a total of 5 top outs (523 sacks), cement returned to surface and remained static. |
1508942263 | 2017-10-25 | Drill / Deepen / Reopen | No Violations | On the day of my inspection Patterson 284 drilled 10 below the set mine string to a depth of 234. The hole was loaded with water and watched for 30 minutes. No fluid loss was noted. Hydrostatic test was successful and they can continue drilling for surface casing. |
1655492087 | 2017-10-30 | Drill / Deepen / Reopen | No Violations | On the date of my inspection I found Patterson rig 284 drilling the intermediate section of this well at approximately 6340 feet. There are a few tears in the containment near the cutting loading area as well as the area near the processed water load out. There is some oil based mud on the ground outside of containment near the processed water load out. The Ascent company man indicated that he would have the containment repaired and the mats pulled so the oil based mud could be removed from beneath them to stop trucks from tracking it onto the gravel. All of the chemicals were within containment. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking. The Ascent company man indicated that the pad drain on the south side had failed the chloride test and would be hauled until the concentration went down. |
1972475242 | 2017-10-26 | Casing String Witnessed | No Violations | I am on site to witness the cementing of 13.375 inch surface casing. Patterson Drilling rig 284 drilled a 17.5 inch diameter hole to a depth of approximately 950 feet and set 950 feet of 13.375 inch 61 pound per foot casing. Schlumberger established circulation with fresh water, pumped 35 barrels of gel ahead, then pumped 830 sacks of class A cement, 1.2 yield and 15.6 pound per gallon, Surface Set. Schlumberger displaced with 137 barrels of fresh water. Approximately 20 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 925 psi for 5 minutes. |
