| On the day of my inspection I witnessed that Highlands Drilling Rig #10 drilled a 23 inch diameter hole to the depth of 560 feet and ran 529 feet of 18.625 inch 87.5#/ft. J-55 casing. While running the last joint a rig hand got his finger caught between the casing and the tongs. Job was shut down for stiches and x-rays which revealed a fracture on his right middle finger. Safety meeting was held and then Highlands Drilling Rig #10 stabbed the float shoe with 4.5 inch diameter drill pipe. Baker Hughes was on site and broke circulation and pumped 35bbls of gel, 10bbls of fresh water and cemented casing with 560 sacks of Class A lead cement with 0.05% Static Free, 1% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, 0.5% BA-10A. Lead cement was followed 155 sacks of Class A tail cement with 0.05% Static Free, 3% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, and 0.5% BA-10A. Displaced cement with 7.1bbls of fresh water. There were good cement returns of 45bbls to surface. While Baker-Hughes was rigging down the pump truck driver was disconnecting a stuck suction hose when the hose released it hit his mouth resulting in a tooth being lost.

The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. Roustabouts were on location assembling production lines for production equipment.

1054786066 2016-09-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed no current activity and the drilling permit expired on 6/24/16. The last activity was on 10/22/14 with the 13.375 inch intermediate being set at 1507 feet.
1413772908 2014-10-20 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Highlands Drilling Rig #10 drilled a 23 inch diameter hole to the depth of 560 feet and ran 529 feet of 18.625 inch 87.5#/ft. J-55 casing. While running the last joint a rig hand got his finger caught between the casing and the tongs. Job was shut down for stiches and x-rays which revealed a fracture on his right middle finger. Safety meeting was held and then Highlands Drilling Rig #10 stabbed the float shoe with 4.5 inch diameter drill pipe. Baker Hughes was on site and broke circulation and pumped 35bbls of gel, 10bbls of fresh water and cemented casing with 560 sacks of Class A lead cement with 0.05% Static Free, 1% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, 0.5% BA-10A. Lead cement was followed 155 sacks of Class A tail cement with 0.05% Static Free, 3% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, and 0.5% BA-10A. Displaced cement with 7.1bbls of fresh water. There were good cement returns of 45bbls to surface. While Baker-Hughes was rigging down the pump truck driver was disconnecting a stuck suction hose when the hose released it hit his mouth resulting in a tooth being lost.
