| Well is equipped and capable of production.  The well has an annular disposal connection that has been used.  There are numerous buckets on site with fluid in them. The identification is incomplete. There is pollution and contamination around the well and in the tank dike.  Tank battery is too close to the well.  It is less than 30 ft.  The lease road for this well needs graded.  There are 1 to 1.5 ft. ruts in it.  Production has not been submitted for the year 2011.  All connections from the brine tank to the well need to be removed as well as the connection from the production tubing to the 4.5 in. annulus.  All pollution and contaminated fluids and soils need removed from well site and disposed of at an EPA approved landfillComplete identification needs posted at well head. Tank battery needs to be moved to an appropriate distance from the well head.  The minimum requirement is 50 ft.  Production records need submitted for this well.  Failure to comply will result in forfeiture of bond. | An inspection was conducted with Oil & Gas Supervisor, Mr. Robert Stonerock and Mr. Paul Sinkovitz.  It was noted that Mr. Paul Sinkovitz is placing brine back back through the casing into the well bore.  Mr. Sinkovitz stated that he floats the oil out, which also alllows for him to continuously pump his well.  Mr. Tom Tomastik, Geologist, Technical Support Services Section and Mr. Jeff Fry, UIC Field Supervisor were then contacted and stated that this operation is considered to be illegal saltwater injection.  On 5/1/2009, an inspection conducted by Mr. Stonerock and Mr. Fry confirmed illegal saltwater injection  Both Mr. Stonerock and Mr. Fry informed Mr. Sinkovitz, in the field, to disconnect lines to the well for this operation.

-610037065 2009-04-23 An inspection was conducted with Oil & Gas Supervisor, Mr. Robert Stonerock and Mr. Paul Sinkovitz. It was noted that Mr. Paul Sinkovitz is placing brine back back through the casing into the well bore. Mr. Sinkovitz stated that he floats the oil out, which also alllows for him to continuously pump his well. Mr. Tom Tomastik, Geologist, Technical Support Services Section and Mr. Jeff Fry, UIC Field Supervisor were then contacted and stated that this operation is considered to be illegal saltwater injection. On 5/1/2009, an inspection conducted by Mr. Stonerock and Mr. Fry confirmed illegal saltwater injection Both Mr. Stonerock and Mr. Fry informed Mr. Sinkovitz, in the field, to disconnect lines to the well for this operation.
