0243572781 2009-04-23 Annular Disposal Violations Noted An inspection was conducted with Oil & Gas Supervisor, Mr. Robert Stonerock and Mr. Paul Sinkovitz. It was noted that the earthen dike around the well head contained brine. Mr. Stonerock utilized High Range Quantabs and verified that chlorides exceed 250 mg/l. Mr. Stonerock informed Mr. Sinkovitz that he could place brine back into the tank. Mr. Sinkovitz was also informed that Mr. Brent Bear would be conducting a follow-up inspection. Note: Production water is being placed back down the well. The UIC section will be contacted regarding this inspection.
0610037065 2009-04-23 Annular Disposal Violations Noted An inspection was conducted with Oil & Gas Supervisor, Mr. Robert Stonerock and Mr. Paul Sinkovitz. It was noted that Mr. Paul Sinkovitz is placing brine back back through the casing into the well bore. Mr. Sinkovitz stated that he floats the oil out, which also alllows for him to continuously pump his well. Mr. Tom Tomastik, Geologist, Technical Support Services Section and Mr. Jeff Fry, UIC Field Supervisor were then contacted and stated that this operation is considered to be illegal saltwater injection. On 5/1/2009, an inspection conducted by Mr. Stonerock and Mr. Fry confirmed illegal saltwater injection Both Mr. Stonerock and Mr. Fry informed Mr. Sinkovitz, in the field, to disconnect lines to the well for this operation.
0966748287 2018-08-07 Production Wells No Violations I conducted a compliance notice follow-up on this well today. Inspector Andrew Thomas issued a compliance notice on 3/21/2013 for multiple violations. The well was equipped with a pumping unit and motor. I did not observe pollution and contamination at the well. The tank battery has not been moved further from the wellhead. The well did not show signs of recent production as the polish rod was dull. The last year for reported commercial production was 2010. The tank battery consists of (2) 100 barrel tanks. I observed identification on the ground at the well but not at the tank battery. I took photos of the well and tank battery. The well is not in compliance. Time of inspection: 10:50 AM. Weather: 80 degrees F, Sunny
1067853436 2007-03-13 Production Wells No Violations A follow-up inspection revealed that the previously noted violation has not been resolved. During the inspection, it was noted that the dike for this tank battery does not meet SPCC requirements. Although work was performed on this dike, only a section located near the road was addressed. The owner, Mr. Paul Sinkovitz, stated that he would not be doing any more work to this dike. Mr. Sinkovitz also stated that he has already spent enough money and would have to disconnect production lines to allow access to the remaining area surrounding the tank battery. The area of concern for this tank battery is the flat terrain leading to a steep slope. There is no dike to prevent a discharge from oil/gas related activity or from a rainfall event. Noted during all inspections was that the production area shows signs of brine discharge. Bare ground has been observed around production equipment and also over the hill. Mr. Sinkovitz was instructed after the initial inspection to seed and mulch area and to properly dispose brine. The dike still needs pumped and oily soil removed from the lease.

All connections from the brine tank to the well need to be removed as well as the connection from the production tubing to the 4.5 in. annulus. All pollution and contaminated fluids and soils need removed from well site and disposed of at an EPA approved landfill. Complete identification needs posted at well head. Tank battery needs to be moved to an appropriate distance from the well head. The minimum requirement is 50 ft. Production records need submitted for this well. Failure to comply will result in forfeiture of bond. Accompanied by Southwest Region Supervisor Dan Goins.

During the inspection, it was noted that a rubber line that allows the placement of brine back into well has been cut at the well. It was noted however, lines are still attached to the tank and plug has not been placed inside the line. Informed Mr. Sinkovitz that management with DMRM had wanted a plug to be placed inside the water lines. Mr. Sinkovitz responded by stating that the line will not leak because the valve has been closed.
