-1150597566 2014-12-05 I was onsite with Regional Supervisor Tom Hill to conduct a preliminary restoration inspection. There is one well on the pad. Access road entry is on north side of SR 558 approximately 0.5 miles west of Grafton Road. There is a sign board at the entrance featuring the well names, permit numbers and emergency contact information. There is a padlocked gate at the entrance of the access road. The access road is in good repair, and drainage and erosion controls are in place. Ground cover is well established along the access road. I found the drilling/production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. The pad has a crushed limestone surface, and is in good repair. There is no drainage system for the pad installed. The well is constructed with a
1359643883 2012-08-27 I went to the site to review the construction process of the drilling pad. Construction was complete. I noted that the soil stockpile has been stabilized.
1359750252 2012-11-28 I was at the site to check on the drilling progress on the drilling of the mine string hole. Nomac 245 was drilling slowly at 174'. If a mine void is to be encountered, it is estimated to be at approximately 228' KB.
1360089943 2012-11-29 I went to the site to witness the cementing of the mine string casing. The drilling contractor, Nomac 245, spudded the well on 11/28/2012. The 24.00
-1389608687 2012-09-17 I witnessed the cementing of the conductor casing. United Drilling drilled a 30 diameter hole and installed 30 OD diameter casing into hard rock at 42', leaving no annulus. United then drilled a 28 diameter hole to 120'. United then installed 120' of 26 OD casing. During drilling, water was encountered at an approximate depth of 50'. Allied Cementing Company was the cementing contractor. The cement was mudscaled at 15.2 pounds per gallon (ppg). Allied adjusted the density, and pumped 34.0 bbls of cement slurry at 15.6 ppg. The cement was displaced with 71.9 bbls of water. Cement returned to surface at the end of displacment. Displacement was completed at 1:28pm. The cement fell about 2' in one hour. United will let the cement set for approximately 8 hours, and then tag the top to see if additional cement will be needed to bring the top to the surface of the cellar.
1406576588 2014-05-20 I reviewed the site to assess the completion of preliminary restoration. There is a sign board at the access road entry featuring the well name, permit number and emergency contact information. The access road is in good repair. Drainage and erosion controls were in place and appear to be functioning. Ground cover was well established along the access road. I found the drilling/production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. The pad has a crushed limestone surface, and is in good repair. There is some standing water along some of the pad edges. I walked the exterior perimeter and noted that the berm is in good repair and ground cover is well established. The area of disturbance around the pad has good ground cover. Crops are growing well outside the area of disturbance. The soil stockpile is well stabilized with vegetation. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The well has been hydraulically fractured, and has a Christmas tree valve assembly. The well is not connected to the GPU units at this time. The tank battery consist of two 400 barrel tanks. There are bull gates and concrete barriers surrounding the wellhead and cellar to prevent vehicle impacts. There are grates over the cellar to prevent falls. There is no ID tag on the well, but it is the only one on the site. The cellar is full of water to the top. The mouse hole has not been plugged, but has a steel plate welded on top. Preliminary restoration approval will not be granted pending installation of pipeline connecting the well to the GPU units. The area is also used as storage for some pipeline equipment.
-1522875300 2012-11-26 I went to the site to check on the rigging up process and arrange for a pre-spud meeting. I spoke with Greg Stutzman, the tool pusher, and Kevin Cartwright, a mud engineer. They contacted the drilling consultant, Clay Chauviere, and arrangements were made for a pre-spud meeting tomorrow, 11/27/2012.

-1778090160 2012-09-13 I was at the site checking the drilling progress. United Drilling was onsite drilling the hole for the conductor pipe. The cellar had been dug, and United had installed 30 OD casing to 37'. It was installed so there was no annulus. Drilling was completed for the day. United's next step is to drill a 28 diameter hole to 120', and install 26 OD casing to total depth.
-454151769 2012-11-23 I went to the site as drilling rig Nomac 245 was moving in. Mat boards were down, and truckloads of equipment were arriving. The rig itself was not yet on site.
-490414246 2012-12-12 I was at the site checking the drilling progress. Nomac 245 was at 6,700
-495143426 2012-09-18 Jimmy Freeman of Chesapeake checked the depth of the top of the cement from the conductor cement job done yesterday (09/17/2012). The top of the cement was tagged at 13' below the floor of the cellar. Allied Cementing Service returned to the site, and ran 1 pipe to a depth of 6' below the cellar floor in the annulus between the 30 casing and the 26 casing. Ten bbls of cement slurry at 15.9 lbs per gallon were pumped through the 1 pipe. Cement was circulated to the surface of the cellar, and provided a 1.5' thick cellar floor. The top out job was complete at 12:58pm.
582954029 2012-11-27 I met with Clay Chauviere, a drilling consultant, and Tim Cantrell, a drilling superintendent, for Nomac 245 and Chesapeake Exploration, and had a pre-spud meeting. We discussed the requirements regarding the drilling and installation of a possible mine string at the site.
866897996 2012-12-01 I went to the site to witness the cementing of the surface casing. The 17.50' diameter surface hole was drilled to 630'. There were no issues while drilling the surface hole. The 13.375 OD surface casing was set at 630' KB. A total of six centralizers were installed on the surface casing string. Schlumberger was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.6 lbs per gallon (ppg). Schlumberger pumped 220 bbls of cement slurry at 15.6 ppg. Cement returns to surface were observed, and were mudscaled at 15.6ppg. An estimated 80 bbls of cement returned to surface. The plug was displaced with 84 bbls of water. The plug bumped the float collar at 4:29pm. Schlumberger pressured up on the casing, and the pressure held. When the pressure was released, approximately 0.6 bbls of water flowed back.
