| I conducted an inspection on this well.  The well was equipped with a plunger lift assembly and in production, at the time of this inspection.  The well was located within the fenced tank battery location.  The tank battery was comprised of one 100 BBL tank, one 25 BBL tank and a mechanical separator.  Valves on the separator were open and the chart on the sales line showed production.  The 100 BBL tank was equipped with a spark arrestor, correct lids and hatches.  A charcoal filter was plumbed off the vent stack of the production tank.  The tank battery was fenced  and locked with legible identification.  The lease access was stable.

| I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger lift system and associated tank battery. The production elements were co-located in a chain-link enclosure which was locked and secure. Fittings and connections at the wellhead and battery were secure and free from signs of spills, leakage, or other problems. The secondary containment was intact. The production system is located within a commercial gravel and aggregate mining and storage facility. Areas to the south and west have been mined and there is a significant cut-bank near the well. There is a significant topographic drop-off to the east. I walked the areas surrounding the well and found no erosional deficits or signs of degradation. Access to the well is afforded by an unpaved path through the facility which is shared with the facility for parking and materials storage. Access to the well and battery at the time of this inspection was unimpeded and from problems.

| Tank battery has been constructed one 20BBL plastic tank and one 110 BBL tank on site.  Mechanical separator.  Well head and tank battery are fenced.  Gate is locked.  Site around the well is a sand and gravel pit that is active.  Mining had taken place before well was installed.  Area around gas well is also affected area from mining.  Mining operator also has reclamation responsibility in the area.  Tank battery and well head sign is correct.

0746467134 2019-01-14 UrbanProduction Wells No Violations I conducted an inspection on this well.
1247875253 2009-10-20 Production Wells No Violations There were no violations at the time.
1296572528 2011-01-04 Preliminary Restoration No Violations Preliminary restoration meets all requirements.
1314794415 2011-08-31 UrbanPreliminary Restoration No Violations Fencing, ID, dike in compliance. Lightning arrestor and charcoal filter installed. Vent hatches installed and closed. Feedlines are bullplugged. Site passes Preliminary Restoration.
1502637671 2017-02-09 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity.
1634696515 2008-03-24 Production Wells No Violations Tank battery has been constructed one 20BBL plastic tank and one 110 BBL tank on site.
