0089936628 2018-03-29 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains three 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 400 pounds on the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. A gauge for the inside of the 13.375-inch diameter casing and the outside of the 9.625-inch diameter casing is not currently provided. The production records within RBDMS are currently updated to 2017.
0090392135 2018-10-18 Production Wells No Violations On the day of my inspection accompanied by ODNR Southeast District Supervisor Mike Moreland I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing with no artificial lift technology. At the time of my inspection annular pressure monitoring was in place and the 9.625-inch diameter casing was at 725 pounds per square inch, the 13.375-inch diameter casing pressure was 360 pounds per square inch. Production equipment included a Gas Processing Unit, and 3 400-barrel steel production fluid tanks that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
0579209783 2015-12-15 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig, Patterson 296, is drilling a 12.25 borehole at a depth of 3426. The location has not yet been covered with stone and is covered with a layer of mud. Fire extinguishers are staged around location.
0632985870 2015-12-02 Casing String Witnessed No Violations On this day the 36 open hole had reached total depth. The 30 conductor pipe was ran to a depth of 100. Express energy was on site to cement the pipe with 275 sacks of Class A cement. See cement ticket for details.

1355864101 2016-01-21 Drill / Deepen / Reopen No Violations On my inspection today, Patterson Drilling Rig 296 has reached total measured depth for the well at 17,621 feet and is currently pulling drill pipe out of the hole with the bit depth at 14,778. They are, however, experiencing difficulty pulling the pipe, with the weight on the drill string at times exceeding their standard preferred limit, and the process is going very slow. A walk-around inspection of the well pad shows that the secondary containment is in place around the rig, all auxiliary equipment, and chemicals however the foam berm is somewhat flattened at the edge of the rig mats on the two corners on the far side of the pad due to traffic around the rig. The site manager was advised and is aware.
1463299523 2015-12-11 Casing String Witnessed No Violations I am onsite to witness the surface casing installation and cementing operations. The rig tried to established circulation, but was unable to until they pumped a 300 barrel LCM sweep. Once circulation was established Baker Hughes cemented the casing in place with 1021 sacks of class A cement. Circulation was maintained through the job, but not at full rate. The company was instructed to top off the cement through a 1 parasite string.
1551110485 2019-02-25 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The erosion control barriers have been removed and the lease road along with the well pad surface is stone construction and in good repair. There are (2) wells on the pad in the production operation stage. Annular pressure monitoring is in place on the well head A&B sections with production trees on both well heads. Emergency shut down devices are present on the production trees. Signage is posted at the well head and production tanks. There are (3) steel construction production tanks present capable of holding 400 barrels each. The tanks are situated inside a polymer lined steel containment dike area. (2) gas processing units are also present on location. The well pad perimeter is protected by an earthen berm. Emergency shut down devices are situated at all critical points are on the well pad.
