1859642792 | 2014-07-14 | Administrative Inspection | No Violations | At 10:42 AM I called Ms. Smith, the owner of Smith and Smith Oil and Gas. I reminded Ms. Smith that she must get registered and bonded to operate wells in Ohio. Ms. Smith stated that she was working with the Bonding Section and that she has filled several of the documents needed to get registered. |
1892480125 | 2019-03-18 | Production Wells | No Violations | The purpose of my inspection was to follow up on a previously issued compliance notice. On the day of my inspection I witnessed production equipment included a 100-barrel steel production fluid storage tank, and a 250-gallon plastic storage tank contained within an earthen dike. The well was equipped with 7-inch surface casing, 2.375-inch diameter tubing with a gas packer system, rods, a pumping unit and a gas motor, it appeared that casing pressure was being vented into atmosphere. At the time of my inspection I did not witness any signs of dumping of production fluid, the wells were under a historic owner in RBDMS and a violation was written in 2012 for failure to register and maintain boding and insurance. Identification was located on the production equipment and production has never been reported for this well. |
