| I inspected this well location pursuant to a request from a prospective purchaser of the property. At the time of this inspection, I observed the following: a 6-inch outside diameter well casing with 3-bolt lid and domestic-supply nipple located within a subsurface vault with a 30-inch circular cast-iron access lid. I did not note any gas odors or meter-detectable gas within the vault or nearby. It was not known whether there is an active connection to the house located on this parcel. It has been reported that the owner may be deceased. I contacted RA Gibson, local oil and gas contractor, to inquire about any knowledge he had about the well. He reported that the property owner has contracted with him to plug and abandon the well. An application for plugging was reported to be forthcoming.

| I was on location to witness plugging and abandonment. At the time of this inspection, I observed the following: a 6-inch diameter wellhead with 3-bolt lid in a round, masonry vault structure with a cast iron lid. The well was being prepared for plugging and abandonment. Preparation activities were performed by the plugging contractor (RA Gibson). Residual gas was vented and water was bailed from the well. Plugging was executed by top-loading with 9-sack grout, from TD to surface. A detail of these activities can be found in the associated plugging report. Plugging and abandonment was commenced and completed on the same day.

| I inspected this well as a follow-up to plugging and abandonment activity which was completed  on 02/22/2017. At the time of this inspection,  I observed the top of the cement plug to be 3 feet below the top of the casing and stable with no gas or fluid leakage. Final restoration activities to be completed will include attachment of a metal plate with ID and burial of the casing under at least 30-inches of cover.

| I inspected this former well location as an evaluation for Final Restoration following plugging and abandonment which was completed on 02/22/2017. At the time of this inspection, the former vault had been partially dismantled, leaving the  top of the well casing at least 30-inches below the existing grade. The area around the well was undergoing development for residential construction. There were no other known production appurtenances associated with the well at the time of plugging. No other activity is needed. The requirements for final restoration have been met.
