| Preliminary restoration is past due. The grading has been completed but the vegetation cover is thin. The landowner has been contracted by ISSI to complete the project. I explained to Tim Levengood that regardless of the contractor, ISSI is responsible to meet any and all of the deadlines. He will meet with the landowner and discuss completing the project as soon as possible.

| A track hoe and two Chem Tech trucks were on site to start pit solidification operations. Northeast Water Service was also on site to remove any free water prior to the dust application. According to Tim Levengood, a third load of dust was delivered and applied to the pits before the appropriate density was achieved. The pits will be covered over tomorrow.

| As a result of the five inch rainfall that occurred on 7/19, the affected area around this drilled location drained into the landowners pond. A small amount of oil, reportedly less than 5 gallons washed from the frac tanks, was released onto the location and into the pond. The landowner Clarence Lucas, notified the Uniontown office. ISSI responded with booms and sorbent pads to recover and contain the oil.

Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0144536782 2012-07-10 Preliminary Restoration No Violations Preliminary restoration is past due.
0250489682 2011-08-02 Urban Drill / Deepen / Reopen No Violations The oil release has been remediated and work has begun on grading the site for preliminary restoration.
0434110610 2011-06-02 Urban Drill / Deepen / Reopen No Violations Poulson Drilling is moving on location and will spud the well later today. The required identification is posted and the pits have been dug and lined. No violations at this time.
0434950797 2011-06-20 Urban Drill / Deepen / Reopen No Violations The drilling pits have been pumped down and are scheduled to be solidified on June 22. No violations at this time.
1111382512 2011-06-09 Urban Drill / Deepen / Reopen No Violations Poulson is moving off location. The pits are fenced, lined and in good condition. They are full and according to Tim Levengood, a water truck is enroute to start removing pit water.
1308599120 2011-06-07 Urban Drill / Deepen / Reopen No Violations Poulson is drilling ahead and will TD the well later today. The pits are fenced, lined and in relatively good condition. Pit capacity is becoming an issue and water management may be necessary when cementing long string.
1309466162 2011-06-22 Urban Drill / Deepen / Reopen No Violations A track hoe and two Chem Tech trucks were on site to start pit solidification operations. Northeast Water Service was also on site to remove any free water prior to the dust application. According to Tim Levengood, a third load of dust was delivered and applied to the pits before the appropriate density was achieved. The pits will be covered over tomorrow.
1339000067 2011-06-16 Urban Drill / Deepen / Reopen No Violations The drilling pits are still open and have not been pumped down. I will call Tim Levengood and request that at least two loads of water be removed before the weekend.
1586030846 2011-06-29 Urban Drill / Deepen / Reopen No Violations The drilling pits have been solidified and covered over. The well has not been completed. No violations at this time.
2021207883 2011-07-20 Urban Drill / Deepen / Reopen No Violations As a result of the five inch rainfall that occurred on 7/19, the affected area around this drilled location drained into the landowners pond. A small amount of oil, reportedly less than 5 gallons washed from the frac tanks, was released onto the location and into the pond. The landowner Clarence Lucas, notified the Uniontown office. ISSI responded with booms and sorbent pads to recover and contain the oil.
