| On this day, I am conducting a compliance notice follow-up inspection. I found the well equipped with 5 inch diameter casing and no pumping unit attached, as indicated in previous inspection reports. The casing valve is closed with no associated flowline hook-ups. I spoke with Mr. Layton, during the inspection, and he indicated that he does not produce the well. There is thick vegetation surrounding the well. Layco reported zero (0) production from 1998-2014. There is no identification at the wellhead.

| The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter production casing, and 1 1/2 inch diameter production tubing. The valves on the wellhead are closed at the time of inspection. The tank battery for this well is equipped with two (2) 100 barrel steel tanks with no identification on the tanks. The well is idle at the time of inspection.

| The inspection this day revealed that this well is equipped with a scissor jack, 4 1/2 inch diameter production casing, and production tubing. The valves are closed on the wellhead for production. RBDMS production records show that Layco reported zero (0) production from 1998 to 2014 on this well and 2015 to 2017 has not been reported. There is no identification posted on the well. I spoke with Mr. Layton and he claims he does not own this well and it is not in production as he has never pumped it. He says the only two of the Oliver lease that he owns and pumps is the Oliver 1 and 2. I informed him I would be issuing a violation to this well for nonproduction and failure to legibly identify the well. There is no identification posted on the wellhead or the tank. In accordance with 1501:9-9-05(A)(10) all wells must be legibly identified in a conspicuous place on or near the wellhead or storage tanks with the owner, lease name, well number, permit number, county, and emergency contact information. This well is not in production and is idle. The well owner/operator shall submit a written plugging schedule to Brian Sefcik at 5880 Memory Road Zanesville, OH 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

0425139702 2018-10-23 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. I found the well equipped with 5 inch diameter casing and no pumping unit attached, as indicated in previous inspection reports. The casing valve is closed with no associated flowline hook-ups. I spoke with Mr. Layton, during the inspection, and he indicated that he does not produce the well. There is thick vegetation surrounding the well. Layco reported zero (0) production from 1998-2014. There is no identification at the wellhead.
1565588004 2007-03-14 Production Wells No Violations Inspection this day with Mr. Layton found well producing with jack, all tied together for house gas. STEVE - this well not on map? Could this group of 5 wells be the wells on map shown south of here or never permited?
1593973670 2017-07-26 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter production casing, and 1 1/2 inch diameter production tubing. The valves on the wellhead are closed at the time of inspection. The tank battery for this well is equipped with two (2) 100 barrel steel tanks with no identification on the tanks. The well is idle at the time of inspection.
1698615330 2018-02-06 Production Wells Violations Noted The inspection this day revealed that this well is equipped with a scissor jack, 4 1/2 inch diameter production casing, and production tubing. The valves are closed on the wellhead for production. RBDMS production records show that Layco reported zero (0) production from 1998 to 2014 on this well and 2015 to 2017 has not been reported. There is no identification posted on the well. I spoke with Mr. Layton and he claims he does not own this well and it is not in production as he has never pumped it. He says the only two of the Oliver lease that he owns and pumps is the Oliver 1 and 2. I informed him I would be issuing a violation to this well for nonproduction and failure to legibly identify the well. There is no identification posted on the wellhead or the tank. In accordance with 1501:9-9-05(A)(10) all wells must be legibly identified in a conspicuous place on or near the wellhead or storage tanks with the owner, lease name, well number, permit number, county, and emergency contact information. This well is not in production and is idle. The well owner/operator shall submit a written plugging schedule to Brian Sefcik at 5880 Memory Road Zanesville, OH 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
