| I was onsite to conduct a preliminary restoration inspection. There are two wells on the pad. Access road entry is on the south side of Lewis Road approximately 0.17 miles east of Emerick Road. There is a sign board at the entrance featuring the well names, permit numbers and emergency contact information. There is a cattle gate that is open midway up the access road. The access road is in good repair, and drainage and erosion controls are in place. Ground cover is well established along the access road. I found the drilling/production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. | The purpose of my inspection was to conduct a status check on well #2-1696.  The access road, pad and erosion and sedimentation controls were in good repair.  The security fence remained in good repair.  Correct identification was present.  The well head was protected by concrete jersey barriers.  The cellar was backfiled with gravel.  Annular pressure gauges were plumbed above grade and identified.  The 9.625 and 5.5 valves were open.  The gauges indicated 0 psi on each valve.  No leaks were observed.  Production equipment on the pad consisted of a well specific sand knock out and GPU.  Common equipment consisted of one (1) hydrate inhibitor tank, one (1) thermal generator, and one (1) meter run and one (1) VRU.  The tank battery consisted of three (3) 400 bbl produced water tanks.  The tank were inside an impervious lined steel containment.  Valves were closed and plugged.  The containment was holding fresh water.  No violations were observed during this inspection.

0275328444 2017-08-30 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1696. The access road, pad and erosion and sedimentation controls were in good repair. The security fence remained in good repair. Correct identification was present. The well head was protected by concrete jersey barriers. The cellar was backfiled with gravel. Annular pressure gauges were plumbed above grade and identified. The 9.625 and 5.5 valves were open. The gauges indicated 0 psi on each valve. No leaks were observed. Production equipment on the pad consisted of a well specific sand knock out and GPU. Common equipment consisted of one (1) hydrate inhibitor tank, one (1) thermal generator, and one (1) meter run and one (1) VRU. The tank battery consisted of three (3) 400 bbl produced water tanks. The tank were inside an impervious lined steel containment. Valves were closed and plugged. The containment was holding fresh water. No violations were observed during this inspection.
0641615450 2013-08-06 Drill / Deepen / Reopen No Violations I was at the site to check the progress on the conductor drilling operation. Precision Rathole was on location and in the process of drilling the conductor hole. A second rig was on site drilling the conductor hole for the Summitcrest 35-14-4 8H. In addition, a crew was on site installing a pad drainage system.
0740723026 2016-03-31 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the

access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #3H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the tank battery. The equipment on the pad consists of the following: 2 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 2 - Gas processing units (GPU) 2 - Sand separators 1 - Thermal electric generator 2 - Producing wells

1139292289 2018-04-05 Production Wells No Violations The purpose of my inspection was to conduct a status check of annular pressures on well #2-1696. The intermediate and production casing annulars were plumbed above grade and identified. The valves were open and both gauges indicated 0 psi. No other leaks were observed. No violations were observed during this inspection.

1445412833 2017-07-14 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1696. The lease access road and pad remained in good repair. I met Chesapeake instrument and electical technician Robby Alexander on location. Contractors were on location installing production instrumentation on the 6H and 10H. Well #2-1696 was currently shut in from production. Both annular pressure gauges were identified and indicated 0 psi. No violations were observed during this inspection.
1462730862 2017-02-01 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1696-00-00. RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. At the time of my inspection, the local area had received a coating of snow that partially covered the location. The lease road was also utilized for farming and remained drivable. No signs of erosion were observed adjacent to the road traveling back to the pad. The soil stockpile was stable with well-

The e-stops are not yet installed outside the barriers. The tanks are grounded, and have static dissipaters. There is no truck grounding area for trucks collecting condensate and/or produced water yet installed. There is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Crops, woodlands and/or underbrush are growing normally outside the area of disturbance. Drainage and erosion controls outside the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.
