| C & G Oil and Gas are the owner/operators of this well. The well is currently in production and is equipped with a pumping unit. There are three 210 bbl tanks up off of SR 376 for this well. There was a call from a hunter who stated that there was an oil line break somewhere in the area. I found the break and it had been repaired at some point in time. The line was not currently leaking oil. There was around a 75' long x 7' wide area of oil contamination leading down the flow line access road. Further down there was another area where the line was repaired, which had a 30' long and 15' wide area of oil. Both spots of oil were on the surface and sprayed on the vegetation. I talked with Bill Grizer and he was heading to the location to clean up the mess. He will remove the oil contamination, lime the area, and then restore vegetation.

| My inspection this day due to request from owner of lease (who is producing well) to resolved the violation.  I have researched RBDMS and have found no violation.  Previous lease by Circleville Gas has been broke and a new lease signed and recorded with C & G Oil who has had service rig on well and it is now producing with a pumping unit.  C & G have tried to contact Bob Ray to get a transfer done but have not been able to contact him (nor could the State DNR after numerous tries).  I have told new operator to try and get the wells in their name - they are continuing to do so but not luck yet.  Big 200 bbl stock tank, laydown separator and current chat in meter with identification saying well belongs to C & G.  This day present as they are selling oil.  Dike does not meet SPCC requirements but progress has been made getting wells back into operation.

| Compliance Notice Follow-up 1191832283: Bill Grizer called and stated that he had cleaned up the majority of the contamination and spread a biodegradable absorbent on the ground. Upon inspection, I found there were still areas that needed attention. There was grass growing in certain areas. I called Bill and discussed the issues and what needed to be complete in order to pass the compliance notice. He agreed and stated that he would be back out sometime near the end of the week.

Upon inspection, I found that Area 1 still has brine contamination in several areas. There is some grass growing but vegetative regrowth is insufficient at this time. Area two has  been cleared of all debris and several trees have been cut and stacked on the hillside. Seed and straw have been spread across area of concern. The black bags used to store the waste are at the top of the road and have begun to leak. I spoke with Bill and he stated that the bags will be gone by the end of the week. I am going to make plans to meet him in order to make sure he is aware of the remainder of the location issues.

| My inspection this day with Jake Glascock, then tell him about some of the problem wells in the county.  We found no violations with this well but it is now being operated by C & G Oil Co and, while they feel they have legal right to operate the wells and Columbus has said to put these in the Orphan well program, C & G will need to get wells on their bond (will go on Carper Well Service bond).  Well is producing this day, good identification, meter house locked so could not check chart, will continue to monitor.
