-465337490 2012-07-17 My inspection of this well was in response to a request from Norm Stokes [ landowner ]. My inspection found there is no tank battery for this well. Further inspection of wellhead found 4.5 casing with swedge reduced to 2 with a valve which is shut-in. The well is disconnected from the flow line. Our database records showed no production reported since 1993. This well is idle. I spoke with Ralph Tallmadge (Northwood Energy) of this idle well violation and the deadline [ 9-21-2012 ] for remediation of this violation.
-789032456 2013-08-15 I arrived on location on 08/15/2013 and Nick's Well Plugging was on location to perform the plug job. Nick's rigged up and bullheaded the bottom plug with 30 sacks of Class A cement and 50lbs of cottonseed hulls. The plug landed and the well was pressured up to 1500psi. The well was shut in until the following morning.
1376655411 2013-08-16 When I arrived on location on 8/16/2013 Nick's Well Plugging was rigged up and ripped the 4.5" casing at 2285'. Nick's then broke circulation on the well and proceeded to run 86 sacks of Class A cement to cover the Big Lime plug. Nick's pulled 61 joints of 4.5" casing. Nick's then broke circulation and proceeded to run 115 sacks of Class A cement. Cement was circulated to surface. The remaining 12 joints of 4.5" casing was removed from the well and the hole was topped off with cement.
1386863152 2013-09-24 The final restoration has been completed . There is a landowner waiver to leave three telephone poles from road to well site area . No violations . The final restoration is approved .
230924419 2013-08-29 The well has been plugged resolving the compliance notice issued on 7-17-2012 . Complaintant [ Norm Stokes ] . NO VIOLATIONS RESOLVED .
