-1120547370	2011-09-06		Met with Kurt Kollar of OEPA he showed me what work was completed on Sunday 09-04-11 and Monday 09-05-11. Then proceed to meeting with City of North Royalton and OEPA to discuss the plans for remediation of the site.( Run a camera into the Storm Water Line along Mercury Drive. and to set up an Oil Separator in on tree lawn between address 3674 and 3664 Mercury Drive and to re-excavation that was excavated on 08-29-11to recheck the flow lines for leaks). Returned to site work started at the graveled parking area of the tank battery. CJ Cutter made a phone called to get a solid material vacuum truck to remove rock ,gravel, soil and fluids from the flow line excavation. Once flow lines were exposed Mr. Cutter performed a pressure test on Flow line used 1000PSI on lines a failure at an elbow was observed no other leaks were observed.		9-1-07

-530866012	2014-07-10		I inspected this producing unit to evaluate conditions for preliminary restoration. The well was completed in 05/17/2010. Elements of the well consist of a production wellhead with standard valve assembly and pump jack system. The wellhead is located in a closed and locked board-on-board enclosure. The 8.625-inch casing is buried and not accessible as required. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located east and adjacent to the wellhead. The battery is comprised of a total of 4 steel 100-barrel production tanks, 2 50-barrel poly brine tanks, and 2 horizontal separators. Spacing from well to battery, and spacing of elements within the battery meet regulations. The battery is shared with the #2-2084 unit. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged and locked. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. The area formerly occupied by drilling equipment, including former pits, has been restored and reclaimed. There is well established grassy vegetation throughout the area. The area of the well is generally level with a slight topographic drop to the east. Surface draining appears to be to the east. There are no surface water courses evident near the site. I did not observe any signs of gully formation or washouts. Access to the unit is afforded by a dedicated gravel-paved access road located directly as an extension from the end of Mercury Drive. The short access road is currently in good condition and free of washout or erosional deficits. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are moderately wooded with single-family residential homes bordering to the east, north and west. The requirements for Preliminary Restoration have passed. I contacted Cutter Oil about the requirement to make the 8 5/8" casing accessible.
-607946946	2011-08-29		Phone call from Mayor Stefanik of oil deposits on the graveled area of the tank battery for two wells P#2059 Callas and P# 2159 Zillich owned by CJ Cutter oil company. Met Fire Chief Mike Fabish at site found two areas 1) approximately 1' wide and 3'long of dark stained soil and 2) a small patch of leafs coated with oil. Along the north edge of the parking area. Called company they arrived to remove these area. Once into the gravel approximately 18" the gravel was coated with oil. This material was removed and deposed of properly. With flow line exposed approximately 100-150 gallons of fluid was removed by a vacuum truck and put into the water tank inside the tank battery. The excavation area was temporarily backfilled with bark mulch an a sump pipe installed for access to excavated area to vacuum out any fluids that accumulated.		9-1-07
-797680019	2011-08-30		Found two areas approximately 12" diameter of oil stained mulch between the tank battery and sump. Advised the clean up crew to remove these areas an to pump out the fluids from the sumps. The two excavated areas were then filled in with bark mulch and saw dust.

The battery is comprised of 4 140-barrel steel production tanks, 2 50-barrel brine tanks and 2 separator units with Dominion metering run. The battery is shared with the #2084 unit. Both the wellhead and battery are enclosed in separate board-on-board fences. The gates and load-outs are locked. ID placard and secondary containment are good. Process safety gear (spark arrestor, low-pressure relief valve, carbon filter) is present. No problems noted.

Well site has been restored to requirements of 1501-9-9-05. All equipment and tanks have been removed. Well site is in compliance with laws and rules and this case file should be considered closed as of this date.
