At the time of my inspection, I found the following: wellhead with pump jack system and associated battery in separate board-on-board enclosures. The enclosure gates were locked and secure. The fittings and connections at the wellhead and battery were secure and free from problems. The tank lids were closed and secure. The interior of the battery, including secondary containment was free from free oil, staining, or other indications of spills, leaks, or problems. The battery is shared with the Callas 2, located nearby. The electrical panels and load line valves were locked and secure. I walked and inspected the areas surrounding the well and battery and found no indications of impacts or problems relating to the on-going production activity. The shared-use access road was snow-covered by accessible.

At the time of this inspection, I noted the following: a wellhead with pump-jack system and associated tank battery in separate board-on-board enclosures. The battery is shared with the Callas #2 located on the same parcel. The enclosure gates were locked and secure. Fittings and connections at the wellhead were secure. The surface casing pressure was being monitored and showed no pressure. The electrical panel box w as locked and secure. Fittings and connections at the battery were secure. The load-line valves were plugged and locked. The tank lids were closed and sealed. The secondary containment drain was secure. The dedicated gravel-paved access road was level and dry. I did note some landscaping fabric debris in the parking area and I notified CJ Cutter about the need for removal of the debris. The entry gate was locked. I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity.

0530866012 2014-07-10 UrbanPreliminary Restoration No Violations I inspected this producing unit to evaluate conditions for preliminary restoration. The well was completed in 05/17/2010. Elements of the well consist of a production wellhead with standard valve assembly and pump jack system. The wellhead is located in a closed and locked board-on-board enclosure. The 8.625-inch casing is buried and not accessible as required. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located east and adjacent to the wellhead. The battery is comprised of a total of 4 steel 100-barrel production tanks, 2 50-barrel poly brine tanks, and 2 horizontal separators. Spacing from well to battery, and spacing of elements within the battery meet regulations. The battery is shared with the #2-2084 unit. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged and locked. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. The area formerly occupied by drilling equipment, including former pits, has been restored and reclaimed. There is well established grassy vegetation throughout the area. The area of the well is generally level with a slight topographic drop to the east. Surface draining appears to be to the east. There are no surface water courses evident near the site. I did not observe any signs of gully formation or washouts. Access to the unit is afforded by a dedicated gravel-paved access road located directly as an extension from the end of Mercury Drive. The short access road is currently in good condition and free of washout or erosional deficits. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are moderately wood
0607946946 2011-08-29 UrbanProduction Wells Violations Noted Phone call from Mayor Stefanik of oil deposits on the graveled area of the tank battery for two wells P#2059 Callas and P# 2159 Zillich owned by CJ Cutter oil company. Met Fire Chief Mike Fabish at site found two areas 1) approximately 1 wide and 3long of dark stained soil and 2) a small patch of leafs coated with oil. Along the north edge of the parking area. Called company they arrived to remove these area. Once into the gravel approximately 18 the gravel was coated with oil. This material was removed and deposed of properly. With flow line exposed approximately 100-150 gallons of fluid was removed by a vacuum truck and put into the water tank inside the tank battery. The excavation area was temporarily backfilled with bark mulch an a sump pipe installed for access to excavated area to vacuum out any fluids that accumulated.

1036679100 2015-02-10 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well and battery are located at the terminus of Mercury Drive. There is a short dedicated access road which travel west from the end of Mercury Drive, and curves slightly northward. The Zilich (#2-2159) is co-located with the Callas#2 (#2-2084) and share a common tank battery. The wellhead pump jack and battery are located in separate board-on-board enclosures. The enclosure gates are locked and signage is present. Observable fittings and connections are secure and free from signs of leakage. At the time of this inspection there is 12 to 15 inches of snow cover which limits my ability to inspect the condition of the lease road, interior of the battery containment, and other surficial features.
1120547370 2011-09-06 UrbanProduction Wells Violations Noted Met with Kurt Kollar of OEPA he showed me what work was completed on Sunday 09-04-11 and Monday 09-05-11. Then proceed to meeting with City of North Royalton and OEPA to discuss the plans for remediation of the site.( Run a camera into the Storm Water Line along Mercury Drive. and to set up an Oil Separator in on tree lawn between address 3674 and 3664 Mercury Drive and to re-excavation that was excavated on 08-29-11to recheck the flow lines for leaks). Returned to site work started at the graveled parking area of the tank battery. CJ Cutter made a phone called to get a solid material vacuum truck to remove rock ,gravel, soil and fluids from the flow line excavation. Once flow lines were exposed Mr. Cutter performed a pressure test on Flow line used 1000PSI on lines a failure at an elbow was observed no other leaks were observed.

The secondary containment was intact, with some water from recent rains. There was no signs of leakage (floating oil or sheen) visible. I walked and inspected the areas surrounding the wellhead and battery and found no signs or evidence of impacts or degradation. The access point was gated and locked with signage present. The access road was in good condition.

The low area of the containment berm have been corrected. Leakage at the brine leakage and oil staining have been corrected and mitigated. Update of signage is in progress. There have been reported complaints that storm drainage from the gravel-paved cul-de-sac at the end of Mercury Drive is causing gravel to wash into the local storm sewer. Reportedly, the cul-de-sac is part of the Callas property. Ive advised Cutter Oil to establish a management solution for the drainage. This work is in progress.

Well site is in compliance with laws and rules and this case file should be considered closed as of this date.

1431717143 2015-06-06 UrbanProduction Wells Violations Noted I inspected the Zillich # 2 - D well on 5/6/2015. The well commenced oil and gas production on 10/18/2010 and is current in reporting production to RBDMS. The access road is off of Mercury Drive and is graveled and offers adequate access. The required signage at the cable gate does not comply with 1501-9-9:05 OAC. The well is operated as an electrified pumping well. The well is fenced with a wooden plank fence.. There is an earthen dike. The access door was locked and secure. The signage, at the well head, does not comply with 1501-9-9:05 OAC. The electric box for the pump jack was not locked. There was not a 8.625 access as required in the Permit conditions.There is a chemical pump and supply drum which was not being utilized at the time of this inspection. If the equipment is not functional and essential to the production of the well the operator shall remove such equipment. A five (5) gallon bucket is full of oil and water and needs removed or emptied and capped, as to not allow accumulation of oil. General housekeeping measures shall be employed, as there are multiple piles of grease and contaminated soils which require removal. The tank battery ( shared with Callas # 2 ) encompasses an area of approximately 50 X 50 and has a wooden plank fence and earthen dike.The signage on the tank battery is not compliant with 1501-9-9:05. The tank battery is absent of the required second door. The production equipment for the Zillich # 2-D included two (2) 140 barrel oil tanks, a 50 barrel poly brine tank, a horizontal mechanical separator, a DOM/EOG gas measurement station and an electric natural gas compressor. The oil, brine load lines and electrical boxes were not locked. The oil storage tanks were equipped with tank lids with seals, but not fastened down. The low pressure vent stacks were present. A charcoal filter was present, but its effectiveness is comprised by an one (1) inch valve at the bottom of the drum. The Operator shall examine the charcoal medium to insure

At the time of this inspection, I met with EOS Energy Representative Mike Corp and North Royalton Fire Chief Chegan. I noted that the well was in active production. The enclosure gates were locked and secure. The identification was current and accurate. The load-line valves were plugged and locked. Tank lids were closed and sealed. I walked the areas surrounding the well and battery and found no signs of problems or indications of impact. I noted the following maintenance items that required attention: I noted that there were some discarded wiring in the battery enclosure which require removal and disposal. I advised Mike to have these items remedied as soon as possible. I also noted that the gate at the access road was not secure and required proper signage. He indicated that he would have the issues addressed as soon as possible.
