We met with well owner Mr. Caslow and readings were taken with ODNR Honeywell PhD6 meter revealing a LEL of 0 and H2S reading of 0 parts per million (PPM). Also utilizing ODNR Tango H2S detector revealed 0 ppm. Proper identification has been posted at the wellhead. Gauge present read 1215 pounds per square inch and was verified with another gauge. Mr. Caslow removed approximately 1 gallon of fluid from the domestic gas drip.

| On the day of my inspection I received a call from ODNR Inspector Pat Shreve who stated he was contacted by Bill Mitton of Knox Energy who had been in contact with the well owner who has concerns with the well and increased pressure. I placed call to Bill Mitton who stated that he has service the well in the past. Two years ago he was onsite to swab the well and was unable to get the rig tools past 4700 feet but the well was making good gas with 500 pounds per square inch (psi) on it. Last year he was onsite and pinholes developed around the 4.5 inch diameter casing slips. Mr. Mitton installed 300 feet of 2.375 inch diameter tubing on a packer. The well had 600 psi on it at that time with Hydrogen Sulfide (H2S0 gas present on the 8.625 inch diameter casing. Mr. Mitton stated that the well pressured up to 1600 psi last week and is currently at 1400 psi with increase production of drip gas. Mr. Mitton was concerned about the increase of pressure of the well on the casing and was wondering if the recent frac of the Donna 2H 34059244280000 could be impacting the well. Upon arrival I witnessed the well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing and domestic gas service. Gauge present read 1390 psi on the tubing and venting of H2S was occurring.  Readings were taken with ODNR Honeywell PhD6 meter revealing a LEL of 4 and H2S reading of 5 parts per million (PPM). Also utilizing ODNR Tango H2S detector revealed 372 ppm. No identification was present. I met with the well owner Bill Caslow and discussed the well status. I stated that the H2S could not be vented into the atmosphere, needs to be controlled and that the well needs to be identified. I also stated that he should not be near the well due to the H2S concentration and that that I am recording the pressures and would review my findings with ODNR Southeast Regional Manager Brent Bear and Ascent Resources Completions Superintendent Jordan Buterbaugh. Also utilizing ODNR Tango H2S detector revealed 0 ppm. Gauge present read 340 pounds per square inch on the casing with valves in the open position and identification present.
1313496023 2017-12-08 Production Wells No Violations On the day of my inspection utilizing ODNR Honeywell PhD6 meter revealed a LEL of 0 and H2S reading of 0 parts per million (ppm). Also utilizing ODNR Tango H2S detector revealed 0 ppm. The well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing set on a packer and domestic gas service. Gauge present read 50 pounds per square inch on the casing. Production equipment has been installed including a 50 barrel tank within an earthen dike and vertical separator. Proper identification was posted at the well.
1423071483 2017-03-14 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Pat Shreve following up after the well was been treated with Sulfa-clear. We met with well owner Mr. Caslow and readings were taken with ODNR Honeywell PhD6 meter revealing a LEL of 0 and H2S reading of 0 parts per million (PPM). Also utilizing ODNR Tango H2S detector revealed 0 ppm. Proper identification has been posted at the wellhead. Gauge present read 1215 pounds per square inch and was verified with another gauge. Mr. Caslow removed approximately 1 gallon of fluid from the domestic gas drip.
1487713848 2017-02-21 Production Wells No Violations On the day of my inspection I received a call from ODNR Inspector Pat Shreve who stated he was contacted by Bill Mitton of Knox Energy who had been in contact with the well owner who has concerns with the well and increased pressure. I placed call to Bill Mitton who stated that he has service the well in the past. Two years ago he was onsite to swab the well and was unable to get the rig tools past 4700 feet but the well was making good gas with 500 pounds per square inch (psi) on it. Last year he was onsite and pinholes developed around the 4.5 inch diameter casing slips. Mr. Mitton installed 300 feet of 2.375 inch diameter tubing on a packer. The well had 600 psi on it at that time with Hydrogen Sulfide (H2S0 gas present on the 8.625 inch diameter casing. Mr. Mitton stated that the well pressured up to 1600 psi last week and is currently at 1400 psi with increase production of drip gas. Mr. Mitton was concerned about the increase of pressure of the well on the casing and was wondering if the recent frac of the Donna 2H 34059244280000 could be impacting the well. Upon arrival I witnessed the well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing and domestic gas service. Gauge present read 1390 psi on the tubing and venting of H2S was occurring. Readings were taken with ODNR Honeywell PhD6 meter revealing a LEL of 4 and H2S reading of 5 parts per million (PPM). Also utilizing ODNR Tango H2S detector revealed 372 ppm. No identification was present. I met with the well owner Bill Caslow and discussed the well status. I stated that the H2S could not be vented into the atmosphere, needs to be controlled and that the well needs to be identified. I also stated that he should not be near the well due to the H2S concentration and that that I am recording the pressures and would review my findings with ODNR Southeast Regional Manager Brent Bear and Ascent Resources Completions Superintendent Jordan Buterbaugh. Also utilizing ODNR Tango H2S detector revealed 0 ppm. Gauge present read 1000 pounds per square inch on the casing. I met with land owners Mr. Bill Caslow Sr. and Mr. Bill Caslow Jr. and they stated that the well has quit producing fluid at this time.

We met with well owner Bill Caslow and readings were taken with ODNR Honeywell PhD6 meter revealing a LEL of 0 and H2S reading of 0 parts per million (PPM). Also utilizing ODNR Tango H2S detector revealed 0 ppm. Proper identification has been posted at the wellhead. Gauge present read 1215 pounds per square inch and was verified with another gauge. Mr. Caslow removed approximately 1 gallon of fluid from the domestic gas drip. The drip gas had a clear appearance and no color. Pressures will be monitored on a monthly basis for health and safety reasons.
