| On this day, I found the well equipped with 20 inch diameter sealed casing at the surface. There is (1) well  in production, and (3) wells that have surface casing set, but were never completed. The inactive wells have sealed casing at the surface. All wells have concrete barriers surrounding them. The facility is gated with proper signage. There is a north pad and a south pad. The south pad contains the wells, along with (2) 400 barrel steel tanks within containment, on the back right-side. The north pad contains the midstream facility, (1) gas processing unit within containment, and (1) 400 barrel steel storage tank, also within containment. There are (2) additional 400 barrel steel tanks within containment, along County Road 16, near the lease road entrance. (Lat. 39.70921 Long. -81.25219) It will be noted that I spoke with Antero representative, Jonah Turner, during my inspection, and informed him to make sure that his operators have their wellheads identified with proper signage.

| The said well was spudded on 05/02/2014 by Zigenfoose Inc top hole rig with 100' of 24'' conductor being set with no annular space, on this day I found the current drilling depth to be 440' using a 22.875'' diameter drill bit in the surface borehole well bore section with a projected total depth to be drilled of 500'. I shall GPS the well head when we cement the surface pipe.

| On 05/08/2014 494' of 20'' .375'' well surface pipe was set and cemented to surface by Baker Hughes cement services using 507 sacks of class A 2% calcium .25 pound per sack cement, the mix water was tested and showed no chlorides above 250 parts per million, with good cement returns to surface. Due to restrictions within the working area of the rig mast the cement head would not fit and no wiper plug was ran, the casing shall be pressure tested at a later time.
