| Status check inspection on the work over process found the well stimulation process complete and Fortis Well Service work over rig on location drilling out the completion plugs. I discussed with the company men on site in regards to the open top 500 barrel flow back tanks allowing a fluid mist to be omitted, they had the fluid hand reduce the flow back pressure through the choke valve thus controlling the mist.

|  Preliminary restoration status check inspection finds the affected area limits of disturbance indicated on the pre-site plans has been properly restored  under ORC 1509.072 with adequate vegetation - erosion & sedimentation control in place. Addressing the well pad working area there is 3 completed wells in production on location with a Patterson Drilling rig 343 on location drilling additional wells on the pad with a primary earthen containment berm in place to control the surface fluids by the use of perimeter drainage sumps and valves with the drilling rig and support equipment being all placed on secondary polymer containment. Located on the North side of the well pad is the production fluid storage tank battery with a total of 8-300 barrel tanks located in a poly lined metal containment dike area. On the Southern side of the well pad the gas processing and monitoring units are present. Preliminary Restoration passed on this day.

0045085325 2014-07-31 Completion Testing No Violations Status check inspection on the completion process found Renegade wireline services running in the hole with a plug and wireline gun with a completion plug to be set at 16,649 and 60 perforations to shot in 6-10 shot burst between 16,492 and 16,639 with Baker Hughes stimulation services on location standing by to stimulate the well.
0128302277 2014-09-16 Completion Testing No Violations Status check inspection on the work over process found the well stimulation process complete and Fortis Well Service work over rig on location drilling out the completion plugs. I discussed with the company men on site in regards to the open top 500 barrel flow back tanks allowing a fluid mist to be omitted, they had the fluid hand reduce the flow back pressure through the choke valve thus controlling the mist.
0636243120 2015-08-05 Preliminary Restoration No Violations Preliminary restoration status check inspection finds the affected area limits of disturbance indicated on the pre-site plans has been properly restored under ORC 1509.072 with adequate vegetation - erosion & sedimentation control in place.
0731040498 2014-04-01 Casing String Witnessed No Violations 04/01/14 the 2nd intermediate 12.250 well bore TD has been reached at 7442 with 9.625 casing ran and proper centralizer placement demonstrated with Allied cement services on location to cement the casing using a lead and tail cement mix.
1181657255 2014-05-30 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found the current drilling depth to be 14,856 in the horizontal well bore section using a 8.5 diameter drill assembly with a current directional azimuth of 156.32 degrees with 2600 units of 3rd party gas.
1393180991 2014-02-23 Drill / Deepen / Reopen No Violations The said well is to be spudded on this day with 100 of 26 weld joint casing to be set inside a 32 borehole.
1393272934 2014-02-24 Casing String Witnessed No Violations The said well was spudded on 02/23/2014 by Express Services top hole rig with 100 of 26 casing set and cemented on this day, the cement lost circulation during the initial job and fell back to 80 measured depth from surface, a coal seam was encountered at 80 while drilling the borehole. On 02/24/14 the casing was grouted through a 1 grout tube to surface by Express Energys cement services with good cement to surface and a verified cement weight of 15.6#.
1393450645 2014-02-26 Drill / Deepen / Reopen No Violations Status check inspection found the 24 surface borehole total drilling depth has been reached at 402, The company is currently running a borehole deviation survey and then will run the 18.625 casing.
1393492618 2014-02-27 Casing String Witnessed No Violations On 02/27/2014- 402 of 18.625 surface casing was set and cemented to surface by Allied Cement services using Class A 2% calcium 1/4# flake cement with 100% open hole excess ran and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug with 40 barrels of good cement returns to surface.
1393866857 2014-03-03 Drill / Deepen / Reopen No Violations Status check inspection found Falcon rig # 20 currently drilling the 17.5 1st intermediate bore hole at a depth of 2,117 with approximately 100 left to drill before the current stage is complete, A borehole directional survey will be run prior to running the production casing.
1395915380 2014-03-27 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found Patterson drilling rig 316 rigged up and drilling the 2nd intermediate 12.250 well bore section with a current total depth drilled of 6,203 and the rig running back in with the drill string after pulling out of the hole to change over to a directional motor to build a 25 degree curve before landing this section at approximately 7,200.
1396276630 2014-03-31 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found Patterson 316 rig on location and the total drilling depth of the 2nd intermediate 12.250 borehole has been reached at a depth of 7,442, the company is conditioning the borehole and preparing to pull out of the hole with the drill string to begin to run the 9.625 2nd intermediate casing string.
1396551954 2014-04-03 Drill / Deepen / Reopen No Violations Status check inspection on drilling process found Patterson rig 316 has finished running and cementing the 9.625 2nd intermediate casing and has skidded to the Keylor # 1-H 3411124449-0000.
1399909948 2014-05-12 Drill / Deepen / Reopen No Violations Status check inspection on the drilling process found the current drilling depth to be 11,280 in the horizontal well bore section using a 8.5 diameter drill bit and 13.9# oil based drilling mud with a current directional azimuth of 149.07 degrees and 1611 units of 3rd party gas, the well bore true vertical depth is 8,411.
1486165581 2019-01-14 Production Wells No Violations On this day, I found the pad gated with signage at the entrance. There are (8) producing wells, (8) gas processing units within steel containment, (8) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present. Additionally, concrete Jersey barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 350 psi.
1569793023 2017-07-17 Production Wells No Violations Status check inspection found the well drilling and completion process now complete and the well in line and producing. There is a gas processing unit on location to support the well along with 8 steel 400 barrel capacity production tanks located inside a polymer lined steel containment dike area. The entry point has signs and gates.
1653398790 2014-03-05 Drill / Deepen / Reopen No Violations Status check inspection found Falcon Drilling rig # 20 has completed the drilling process with the running and cementing of the 13.375 1st intermediate casing string, the rig is moving off location.
