The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. No 13.375 casing well head pressure gauge is present and the 9.625 casing well head reads 1,500 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.

Upon arrival I photographed the site and did a perimeter check finding no drilling fluids had left the site. A remedial clean up procedure was discussed with the affected surface material already being cleaned up and placed into box containers and being hauled to a EPA approved landfill. There was damage to contractor vehicles by flying debris, the vehicles and a mud loggers trailer were also covered by oil based mud. All equipment to be cleaned will be done inside of a containment area and the site will have and surface water and oil base mud vacuumed up and the material hauled to an approved disposal facility for the remainder of the project.

The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. No 13.375 casing well head pressure gauge is present and the 9.625 casing well head reads 1,500 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area. The well lease road entrance is gated and locked with signage posted at the entrance of the well pad and at each well head. The well pad has a stone surface along with the lease road. The well pad perimeter berm is of earthen material and in good repair.

A total depth of 2036 was reached using a 17.50 bit and the company is conditioning the bore hole preparing to run the 13.375 intermediate casing string. The casing tally was reviewed and received noting proper centralizer placement along with a casing shoe and float collar being ran.

The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the back of the pad, and a midstream facility behind the gas processing units.

1366337907 2013-04-18 Drill / Deepen / Reopen No Violations I received a call from the rig superintendent at 11:15 Am requesting me to return to the drilling rig due to a flow line failure casing oil based drilling mud to be discharged under pressure across the South East side of the drill site impoundment. Upon arrival I photographed the site and did a perimeter check finding no drilling fluids had left the site. A remedial clean up procedure was discussed with the affected surface material already being cleaned up and placed into box containers and being hauled to a EPA approved landfill. There was damage to contractor vehicles by flying debris, the vehicles and a mud loggers trailer were also covered by oil based mud. All equipment to be cleaned will be done inside of a containment area and the site will have and surface water and oil base mud vacuumed up and the material hauled to an approved disposal facility for the remainder of the project.

The company will be contacted about having a gauge in place.
