| I was on the phone with Tim Reily from John D, discussing other wells.  I reminded Tim that I was still waiting on a Land Owner Waiver for the lease road on this site.  Tim indicated that the landowner will not sign a waiver.  The landowner was not being reasonable on the amount of final restoration work that they wanted completed.  I informed Tim if the landowner will not sign the waiver, the lease road needs to come out.  Tim advised me that they are short on man power at this time.  I advised Tim to keep working on the waiver.

| I inspected this well location as an evaluation for Final Restoration. A review of file data indicated that the well was plugged and abandoned in 1992 and a plugging report was submitted. At the time of this inspection, I observed the following: The documented former location of the well was observed to be utilised by the current owner for brush colleciton and as a burn pit location.  I observed no evidence of any remaining production equipment or other signs that there had been a well at the location. Final Restoration is complete.

I reminded Tim that I was still waiting on a Land Owner Waiver for the lease road on this site. Tim indicated that the landowner will not sign a waiver. The landowner was not being reasonable on the amount of final restoration work that they wanted completed. I informed Tim if the landowner will not sign the waiver, the lease road needs to come out. Tim advised me that they are short on man power at this time. I advised Tim to keep working on the waiver.

1345747096 2012-08-23 Final Restoration Violations Noted I met with the land owner Joe Ann Tabor on location at 08:45 this morning. I reviewed the location with the land owner and my inspection found the following. The area affected from oil and gas production and plugging operations needs final grading and seeding completed on it. The area of the pit is barren and needs Bio-remediation completed on it to establish vegetation. Bio-remediation will include liming and fertilizing as needed to establish vegetation. I have attached a map to this report designating three areas as A, B and C that still need reclamation work. Please see the attached map. Area A requires: final grading and seeding needs to be completed on it. Area B requires: redistribution of the topsoil,( there is a high area along the south edge of the pad that appears to be topsoil.) This high area needs to be graded out and if feasible used for re-soiling material. The pit area needs liming, fertilizing as needed and re-soiling as needed to establish vegetation. Area C requires: final grading and seeding. There are pieces of pit liner around the location, all trash, debris and large rocks must be removed from the area. 1701824481 2016-08-25 Final Restoration No Violations I inspected this well location as an evaluation for Final Restoration. A review of file data indicated that the well was plugged and abandoned in 1992 and a plugging report was submitted. At the time of this inspection, I observed the following: The documented former location of the well was observed to be utilised by the current owner for brush colleciton and as a burn pit location. I observed no evidence of any remaining production equipment or other signs that there had been a well at the location. Final Restoration is complete.
