| As I was unable to be at the cementing of the production casing, I stopped by the site to discuss the cement job and pick up the associated paperwork from drilling consultant, Terry Getz. The consultant was at the next location for the rig. Personnel and equipment were on site to rig down and move the rig to the next location. The rig move was in progress. I called Mr. Getz, and made arrangements to get the paperwork and discuss the production casing cement job at a later time.

| I went to the site and had a pre-spud meeting with drilling consultant Larry Williamson.  We reviewed the drilling information and production casing installation and cementing data for the Judy Brown 6H (previous well), and discussed the casing cutting and kick-off plug installation for the Altenhof 3H.  Nomac 43 was largely rigged up and waiting on arrival of mud pumps.

| I was notified of the cementing of the production casing.  The drilling contractor, Nomac 43, drilled a 8.5 diameter hole to the total measured depth of 13,680'. There were no issues while drilling. 5.50 OD casing was set at 13,680' KB.  A total of 100 centralizers were installed on the production casing. Schlumberger was the cementing contractor. There was good fluid circulation throughout the cementing operation. The company used a lead cement followed by a tail cement. The lead cement was mudscaled at 13.5ppg. Schlumberger pumped 273 bbls of lead cement at 13.5 ppg. The tail cement was mudscaled at 15.6 ppg. Schlumberger pumped 303 bbls of tail cement at 15.6 ppg. The plug was dropped, and displaced with 302 bbls of water. The plug bumped the float collar at 4:59am on 12/19/2013.  Pressure held. The float held when the pressure was released. 4 bbls of water bled back.

| Inspector Jim Bucksar and I met with landowner, Mr. Tom Altenhof, as a follow-up to a telephone agreement on February 6, 2014, that after the snow melted, I would inspect the area surrounding the access road and drilling pad for erosion and drainage issues.  We walked the entire length of the access road and around the perimeter of the drilling pad.  Photos were taken.  Mr. Altenhof had two major areas of concern.  During my inspection of the first area of concern, I found that along the east side of the northern end of the access road, the drainage ditch became less defined, and run-off from the south was causing erosion and sedimentation that crossed into Mr. Altenhof

| I reviewed the site to assess the completion of preliminary restoration.  There is a sign board at the access road entry featuring the well name, permit number and emergency contact information.  The access road is in good repair.  Drainage and erosion controls were in place and appear to be functioning.  Ground cover was well established along the access road.  I found the drilling/production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate.  The pad has a crushed limestone surface, and is in good repair.  There is some standing water along some of the pad edges.  I walked the exterior perimeter and noted that the berm is in good repair and ground cover is well established.  The area of disturbance around the pad has good ground cover.  Crops are growing well outside the area of disturbance.  The soil stockpile is well stabilized with vegetation.  A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.  There is 25 - 500 barrel frac tanks on location, and has a Christmas tree valve assembly. The tank battery consist of two 400 barrel tanks. The GPU unit is connected to the pipeline. There is no pipelines layed to wellhead.  There are bull gates and concrete barriers surrounding the wellhead and cellar to prevent vehicle impacts.  There are grates over the cellar to prevent falls.  There is no ID tag on the well, but it is the only one on the site.  The cellar is full of water to the top.  The mouse hole has not been plugged, but has a steel plate welded on top.  Preliminary restoration cannot be approved at this time..

| I stopped by to check on the site preparations for the move-in of the drilling rig.  I found that preparations were underway, but no rig equipment was on site.  Some secondary containment was in place and mat boards were down.  A number of frac tanks and light plants were also in place.  One housing unit was in place, and trucks were bringing equipment in as I was leaving the site.

| I was notified of the cementing of the production casing.  The drilling contractor, Nomac 43, drilled a 8.5 diameter hole to the total measured depth of 13,680. There were no issues while drilling. 5.50 OD casing was set at 13,680 KB.

During my inspection of the first area of concern, I found that along the east side of the northern end of the access road, the drainage ditch became less defined, and run-

0110200003 2013-09-03 Drill / Deepen / Reopen No Violations As I was unable to be at the cementing of the production casing, I stopped by the site to discuss the cement job and pick up the associated paperwork from drilling consultant, Terry Getz. The consultant was at the next location for the rig. Personnel and equipment were on site to rig down and move the rig to the next location. The rig move was in progress. I called Mr. Getz, and made arrangements to get the paperwork and discuss the production casing cement job at a later time.

0470516381 2017-02-08 Production Wells No Violations The purpose for my inspection was to conduct a status check well #2-1804-01-00). RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. The lease road is composed of crushed gravel and remains in good repair. No signs of erosion were observed adjacent to the road traveling back to the pad. The soil stockpile remains stable with well-
0520068039 2014-03-20 Preliminary Restoration No Violations ODNR engineer Ron Trivisonno and I met at the site with BJ Morris and others of Chesapeake Exploration, and Mr. Tom Altenhof. Together we reviewed the two areas of concern regarding erosion and sedimentation. Mr. Altenhof expressed his concerns and his desire to get the problems corrected. Mr. Morris and Mr. Trivisonno discussed the issues and possible remedies. Following the inspection, Mr. Morris indicated that he would discuss the problems with Chesapeake engineers, and proceed with developing solutions and correcting the problems. Mr. Morris will keep me up to date with their plans.

1312526130 2013-12-02 Drill / Deepen / Reopen No Violations I went to the site and had a pre-spud meeting with drilling consultant Larry Williamson. We reviewed the drilling information and production casing installation and cementing data for the Judy Brown 6H (previous well), and discussed the casing cutting and kick-off plug installation for the Altenhof 3H. Nomac 43 was largely rigged up and waiting on arrival of mud pumps.
1338819946 2013-12-19 Drill / Deepen / Reopen No Violations I was notified of the cementing of the production casing. The drilling contractor, Nomac 43, drilled a 8.5 diameter hole to the total measured depth of 13,680. There were no issues while drilling. 5.50 OD casing was set at 13,680 KB.

1396465107 2014-03-10 Preliminary Restoration No Violations Inspector Jim Bucksar and I met with landowner, Mr. During my inspection of the first area of concern, I found that along the east side of the northern end of the access road, the drainage ditch became less defined, and run-

1904425251 2014-05-22 Production Wells No Violations I reviewed the site to assess the completion of preliminary restoration.
