The water well shall be properly plugged and abandoned.  Triad Hunter LLC shall submit a written plugging schedule within 30 days to Inspector Lynn Parkins at 5880 Memory Rd. Zanesville, OH 43701 and plug the well within 180 days. Triad Hunter LLC will be notified if the plugging schedule is approved or denied.

| Inspection of this well for Temporary Inactive Status found no items on the checklist that would prevent such status being granted. The tank that was on the location was disassembled. Well was disconnected and had a bull plug in place as to prevent any incidents affecting the environment. Good Identification was found at the wellhead. The well site is a large open pad style layout that has gravel. No safety issues were found to be present. At this time no reason is found to not find this well an acceptable candidate for temporary inactive status.

1289088289 2016-12-27 Administrative Inspection No Violations Inspection of this well for Temporary Inactive Status found no items on the checklist that would prevent such status being granted. The tank that was on the location was disassembled. Well was disconnected and had a bull plug in place as to prevent any incidents affecting the environment. Good Identification was found at the wellhead. The well site is a large open pad style layout that has gravel. No safety issues were found to be present. At this time no reason is found to not find this well an acceptable candidate for temporary inactive status.

Good circulation was achieved throughout the whole job. Good returns were observed at the surface. There were no chlorides detected in the mix water.
1987766182 2013-07-02 Drill / Deepen / Reopen No Violations Rig was shut down again due to the waste handlers. Depth at the time of inspection was 1080 feet. Spoke with Mr. Ray Decker on site. I tested the mix water for the upcoming cement job. It tested at 5.4 with a low range quantab. That means 336 parts per million chlorides. I notified Mr. Decker of this, and he opted to bring different freshwater haulers and test their water for chlorides.
2020751561 2013-06-26 Drill / Deepen / Reopen No Violations Rig modifications and repairs were being made at the time of inspection. Spoke with Mr Ray Decker on site. He stated that rig had sat for 6 months and the hoses needed replaced. They also had to plumb everything up for the closed loop system.
