2687816 Primary Facility 2018-01-25 Drilling/Alteration No Violations Noted Arrive on location: 13:45. Weather: Clear, cold, ~ 20 degs. On location to perform "New Spud" inspection, accompanied by DEP OGI, Ulyana Trynovich. Upon arrival, Express Energy Services has set 20" Conductor and 16" mouseholes for (2) new wells on pad, the Hope 5H and 6H. Both new wells conductors have been set at ~ 80' and cemented to surface. - One (1) existing well was present on pad, the Hope 2H. This well has been disconnected from all previous production equipment and has been temporarily shut in, in preparation for drilling new wells on pad. The 2H well was capped with a 5K master valve. (3) 2" annuli vents were present and plumbed to surface. The (5/P) and (9x5) annuli were shut in to analog gauges, both reading 0 psi. The (13x9) vent was open and venting to atmosphere through a "vent check" fitting. Gas check performed at the (13x9) "vent check" opening showed 0 % LEL, 0 % CH4. Gas check was performed using an MSA Altair 5X gas meter calibrated on 1/22/18. No obvious concerns or violations were note
2716036 Primary Facility 2018-04-04 Drilling/Alteration No Violations Noted Arrived on location: 08:45. Weather: Overcast, light rain, ~ 40 degs. Upon arrival met with Chesapeake "CHK" Reps. Chris Schmaltz, Drilling Superintendent; Terry Hooper, Well-site Super; and Isabelle Leon, Drilling Engineer. The Patterson #572 Rig has recently moved on location to drill (2) new wells on pad, the Hope 5H & 6H. Rig has TD'd the 17 1/2" Surface interval of the 5H well at 550' and is presently running 13 3/8" casing in hole. When set, this surface casing string will be cemented to surface by Schlumberger using their CemCrete blend later this afternoon.

- I confirmed with Mr. Schmaltz that CBLs will be ran on the Surface and Intermediate casing strings of both wells being drilled on this pad to assess cement jobs. I requested a .pdf copy of these CBLs be emailed to me when obtained. - CHK will continue drilling the 5H well to TD prior to skidding to the 6H well. The 20" conductor was set for the 6H well on ~ 1/16/18. - One (1) existing well was present on pad, the Hope 2H. This well has been dis

2720927 Primary Facility 2018-04-17 Drilling/Alteration No Violations Noted Arrived on location: 13:30. Upon arrival I met with Chesapeake "CHK" Co. Man, Terry Hooper. The Patterson #572 Rig is presently RU on the Hope 5H well drilling the 8 1/2" lateral at a depth of 16,882' to a projected TD of 18,342'. Rig has set the 13 3/8" Surface and 9 5/8" Intermediate casings and cemented same using Schlumberger's CemCrete blend. Both these cement jobs went as planned with good returns to surface. CHK ran CBLs on Surface and Intermediate casing strings. Review of these CBLs showed good cement bonding throughout these casing strings.

- CHK will continue drilling the 5H well to TD prior to skidding to the 6H well. The 20" conductor was set for the 6H well on ~ 1/16/18. - One (1) existing well was present on pad, the Hope 2H. This well has been disconnected from all previous production equipment and has been temporarily shut in, in preparation for drilling new wells on pad. No obvious concerns or violations were noted at time of inspection. Left location at 15:15.

2722904 Primary Facility 2018-04-25 Drilling/Alteration No Violations Noted Arrived on location at: 09:00. Weather: Raining, mild ~ 50 degs. Onsite to check on recent surface casing "SC" cement job performed on the 6H well. Yesterday, I received notification of a circulation loss encountered while drilling the surface interval at ~ 350 - 400', via phone call from CHK Drilling Eng., Josh Prater. Mr. Prater asked about the Dept's. regulations regarding the use of loss circulation materials "LCM" through surface interval. I told Mr. Prater that any LCM used should be an environmentally inert material. We further discussed the

possible preventative measures and remedies that may be needed in the event significant cement fallback should occur. I stressed the Dept's. main concern is the protection of the DFGW zone from lower gas bearing zones, specifically the "Zone of Critical Cement", bottom 300' of SC cement, ref. 78.85.(b). Later in day, I received a call from CHK Drilling Eng., Isabelle Leon. Ms. Leon informed that LCM sweeps appear to have been successful and was requesting to alte

2726064 Primary Facility 2018-05-03 Drilling/Alteration No Violations Noted Arrived on location at 10:00. Met w/ CHK Co.Man, Mickey Brewer and Drilling Super., Densiel Bottger. Patterson #572 Rig was RU on the Hope 6H drilling the 8 1/2" Prod. hole at a depth of 8040'. While drilling the original well bore at a depth of ~ 9293', the BHA became stuck in hole. The original BHA was not retrievable and decision was made to sidetrack. This morning at ~ 08:30 the original well bore was successfully troughed off at ~ 8000' TMD. Plan is to sidetrack around the lost BHA and resume the original well bore path to a projected 17,150' TD.

- Since my last insp., Rig has set and cemented the 9 5/8" casing to a depth of 2502'. Due to gas shows encountered in this interval starting at ~ 1375' an external casing packer "ECP" was deployed at ~ 1326'. I reviewed the Intermediate casing cement job log and CBL ran on this string and no concerns were noted. I also reviewed BOP, casing and FIT test charts. All were found in good order. - Rig and location were found in good condition, no violations were n

2731083 Primary Facility 2018-05-15 Drilling/Alteration No Violations Noted Arrived on location at 08:45. Met w/ CHK Co.Man, Mickey Brewer and Drilling Super., Chris Schmalz. Patterson #572 Rig was RU on the Hope 6H well. Rig has TD'd the 8 1/2" Prod. Lateral at a depth of 17,163'. Rig is presently running 5 1/2" Prod. casing in hole at a depth of ~ 10,300'. Crew is working casing through encountered "tight" spot and plans to have casing on bottom later this afternoon and will be cementing same this evening.

I collected, planned, 5 1/2" casing tally detail sheet and reviewed BOP, casing and FIT test charts. All were found in good order. - Rig and location were found in good condition, no violations were noted at time of inspection. The 6H well will be the last of (2) new wells drilled on this pad at this time. Rig will be moving over to the "Joeguswa Pad" in Sullivan County upon completion of drilling on this pad. Left location at 10:15.
