2626876 Primary Facility 2017-08-17 Incident- Response to Accident or Event Pending Prior to conducting the site visit I was informed further details regarding the incident by Eric Hottenstien of Chesapeake Energy. Mr. Hottenstein explained the further investigation estimated the volume of the release was later determined to have been ~2 bbls (84 gallons). The release occurred due to a failure in a valve on the dump line in a GPU (Gas Processing Unit).

I arrived at the site at 12:25. I examined the area where the release had occurred. The area of the spill was behind the McGavin 6HB GPU. The estimated area of impact was ~17' x 30' (~5100 square feet). A vacuum truck was actively managing the fluid while awaiting the arrival of other equipment to conduct excavation of the contaminated material. The material is a fine sediment/gravel mix commonly utilized as well pad surface. It is unlikely that any of the fluid has permeated beyond the berm which surrounds the berm. There was no obvious sign of distressed vegetation. I was informed confirmatory sampling will be conducted by the operator fol

2629969 Primary Facility 2017-08-22 Incident- Response to Accident or Event Violation(s) Noted I arrived at the site at 10:44. The weather conditions were mostly sunny skies with temperatures in the low to mid 80's with a light breeze. There was no activity at the site during the inspection. I met with Nick Krut, Environmental Representative of CHK. I contacted Eric Hottenstein, Environmental Supervisor of CHK on 8/21/17 to inquire about the details of the spill that occurred on 8-17-17. Mr. Hottenstein stated the following information during the phone conversation. "A total of (84) gallons, (2) barrels of production fluid was spilled out of the skid as a part the McGavin 6H E Wyo Gas Production Unit to the pad surface. A crack was discovered in a weld on the skid of the GPU. The result of the spill was failure of a dump valve on the GPU due to a high volume of mesh sand generated from the McGavin E 6H Wyo well as a result of a very high volume of natural gas generated from the well since it has been in production. Hydrostatic testing of the skid unit was not conducted. The skid should have been fille

After reviewing the site plan and performing an inspection, I do not agree that the conditions on site meet the requirements for the termination of this permit. Specifically, the several areas of the pad berm, including the berm breaks pictured in photos 9 and 17, have not attained adequate vegetative cover and contain erodible material under the erosion control blanket covering the berm. As such, I recommend that this NOT application be returned. Operator, please stabilize the aforementioned areas before resubmitting this NOT.

I concluded the inspection and left the site at 1530.
