All wells (13x9) and (9x5) annuli have been plumbed to surface and were tied into a common 2" vent line to production tanks. The (13x9) vents were open and venting w/ SCADA press. monitors. The (9x5) vents were shut in to SCADA press. monitors. All wells (5/P) or prod. csg annuli were shut in to SCADA press. monitors and/or analog gauges. No access was present to perform gas checks at annuli at time of inspection. (See individual well inspections for pressures and further well info.) - The Sharpe Wyo 2H Prod. 2693783 Primary Facility 2018-01-31 Routine/Complete Inspection Violation(s) Noted Weather: Clear, Cold, ~25 degs. (3) wells have been drilled on this pad, the Sharpe Wyo 1H, 2H and 5H. All (3) wells were producing at time of inspection. Inspection of wells showed: Prod. pressures were being monitored by SCADA press. monitor units at prod. tree top and on prod. lines. All wells (5/P) or prod. csg annuli were shut in to SCADA press. monitors. All wells (13x9) and (9x5) annuli were plumbed to surface and tied into a common 2" vent line to prod. tanks "PT". (13x9) vents were open and venting through "Vent Check" fittings w/ inline SCADA press. monitors w/ valve to PT closed. *(9x5) vents were shut in to SCADA press. monitors. A gas check performed at the 2H's (13x9) "Vent Check" fitting opening showed: 100 % LEL, 15% CH4. The sustained gas detected at this cemented annulus is indicative of Defective Cement. No measurable flow was noted at time of insp. (See Violations). I'll return to collect gas sample at later date.
2694794 Primary Facility 2018-02-12 Follow-up Inspection Outstanding Violations - Viols Req'd Upon arrival, Weather: Overcast, Cold, ~32 degs. (3) well have been drilled on this pad, the Sharpe Wyo 1H, 2H and 5H. On location to collect a gas sample off the Sharpe 2H, (13x9) annulus in follow-up to previous insp. of 1/31/18.
  A gas check performed at the 2H, (13x9) annulus confirmed a sustained presence of gas, 100% LEL, 10% CH4. A gas sample was collected off this annulus at 09:45 for Isotopic analysis. Sample will be handed over to DEP Geologist, Eric Rooney for processing. All (3) Sharpe Wyo wells were in production, no production pressures were recorded at time of inspection. Left location at 10:00.
2791249 Primary Facility 2018-10-09 Routine/Complete Inspection No Violations Noted I arrived on site at 1233 to conduct a site restoration inspection on behalf of the Department.

While examining the E&S inspection log, I noticed that no post-storm inspections have been documented on since 8/17/2018. Considering that, according to the National weather Service website, there have been several significant rain events on dates not listed on the inspection log, the Department requests the Operator to determine if any rainfall events have occurred since 8/17/2018 according to their records (aside from the dates listed on the log above) and provide the Department with post-rainfall inspection reports from those dates, or alternatively provide documentation that no rainfall events occurred on the dates between the weekly inspections.

After reviewing the plan and performing a site inspection, I agree that the site conditions meet the requirements for the termination of this permit contingent upon the removal of the rubber mats located in the drainage ditch on the NW side of the pad (shown in phot
