2341745 Primary Facility 2015-01-07 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 12:50. Rick Fields Operations Manager of Warren E&P Incorporated contacted me at 10:20 AM on 1/7/15 to report a flash fire that occurred on this location around 10AM on 1/7/15. Mr. Fields alleged the flash fire was immediately extinguished and there were no injuries as a result of the fire and no fluid was lost from the well. Shortly upon arrival, I met with Rick Davidson, Completions Supervisor of Warren. Mr. Davidson stated that the flash fire occurred at 9:45 AM and was extinguished at 9:46 AM. He concurred with Mr. Fields Statement regarding no injuries occurring as a result of the fire and no loss of fluid. The flash fire occurred as a result of an employee contacting a valve on a control panel on the snubbing unit forcing a puff a gas out of a bleed valve on the snubbing unit.

2633657 Primary Facility 2017-08-23 Routine/Complete Inspection No Violations Noted I arrived at the site at 11:47 AM. There was no activity or personnel at the site during the inspection. The weather conditions are partly cloudy skies and breezy with temperatures around 70 degrees. Warren submitted a stimulation notification to the Department on 7/28/17. The notice indicated that stimulation (hydraulic fracturing) of the Reynolds Unit P&G 4 Reynolds 3H well would commence on 7/30/17 at 12:00 PM. It is apparent that completions activities for this well have recently concluded. Completions activities would have consisted of stimulation, drilling out of hydraulic fracturing plugs and flowback of the well. Some of the equipment used for completions operations remains on the location. This equipment consists of secondary containment, berms for the containment, gas buster tanks and frac tanks. Stormwater from recent storm events has accumulated on the containment liner near the center of the pad near the wellheads, in the frac tank battery containment in the corner of the pad below the site cut

2788024 Primary Facility 2018-10-11 Routine/Complete Inspection No Violations Noted Arrived on site at 13:00 hrs. Weather: ~ 65 degs., light rain. Onsite in response to Application for Inactive Status received for the "P&G 4 P&G 3 - 3H" well on this pad. Upon inspection, a total of (4) wells were present on pad. (3) were in production, the "P&G 4 - 1H", "P&G 4 P&G 1 - 3H" & "P&G 4 Reynolds - 3H".

The "P&G 4 P&G 3 - 3H" well was shut in and disconnected from production line. Well cellar was full of gravel with (2) annular vents plumbed to surface. Both vents were open and venting. The (9 5/8"x 5 1/2") vent was connected to vent tank via stainless tubing. The (13 3/8"x 9 5/8") vent was venting to atmosphere. Gas check performed at the (13 3/8"x 9 5/8") vent opening showed 0% LEL. No obvious concerns were noted at time of inspection. I will be recommending approval of Inactive Status for the "P&G 4 P&G 3 - 3H" well on this pad. Note: The Application for Inactive Status explains that this well is awaiting remedial activities to fish ~ 217' section of stuck tubing from heel of well at ~ 7662'
