2332074 Primary Facility 2014-12-29 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 14:15 in response to an estimated (3.5) gallon spill of production fluid. Heather Burkhart and Kayla Saxon of Warren E&P accompanied me during the inspection. While at the site, Ms. Saxon explained that the spill occurred in the morning hours of 12/19/14 as a vacuum truck was in the process of backing up in preparation to offload production fluid from a frac tank in the tank battery. The truck driver noticed fluid spraying on the pad surface as he was backing up. The spill occurred as a result of a valve failing due to mechanical malfunction from the the scrubber to the muffler line on the truck. Residuals from the truck hose leaked onto the pad surface. The dimesions of the impacted area of the pad surface are (8.5-9.5) in length and (5) feet in width according to Ms. Saxon. Ms. Burkhart and Ms. Saxon informed me that the spill had been cleaned up and backfilled with fresh rock the day it occurred. There is no visual evidence of an impact to the pad surface where the release occurred.

2430040 Primary Facility 2015-11-17 Routine/Complete Inspection No Violations Noted A vacuum truck was in the process of unloading fluid from a frac tank on containment in the tank battery near the pad entrance. The fluid in this tank is either flowback or production fluid from the wells on the pad or a mixture of both. We used an Oakton MultiParameter PCSTester 35, Extech, ExStik2, and a Hanna Soil Test and Electrical Conductivity & Temperature meter to screen the water in the Production Fluid Storage Tanks and Gas Production Unit containment. Conductivity readings were in the range of (400-500) microSiemens/cm³ in water inside the production fluid tanks containment and (200-300) microSiemens/cm³ in water inside the GPU containment. The pH was around 8.4.

2605136 Primary Facility 2017-05-30 Routine/Complete Inspection No Violations Noted I arrived at the location at 13:00. There was no activity or personnel at the site during the inspection. The weather conditions were cloudy skies with breezy conditions and temperatures in the low 60's. Rain had just ended. The Manning 1H, Reimiller 1H and Reynolds 1H wells are in production. I left the location at 13:45.
