1984832 | Primary Facility | 2011-06-28 | Follow-up Inspection | No Violations Noted | Inspection is in response to reported spill. Rig is in process of moving from Solanick to Sickler site.Company Man, Jeff Myers, has MSDS sheets on OB mud in trailer. Night shift topped off mud tank, then in the heat of the day, expansion caused the tank to overflow.Spill was found on 6/27/2011 around 1300. Super vac was here in about 1/2 hour. Total of about 30 gallons over the top with about 5 - 10 gallons splashing off containment. Dirt &fill was dug out and put in waste. Area was about 10' X 10' X 1' deep. Completely cleaned up at this time. Took 3 pictures. Company man had tank level lowerewd immediately so it wouldn't happen again. |

2064936 | Primary Facility | 2012-04-23 | Follow-up Inspection | Pending | Responding to a complaint. Had water-vac truck fill and empty cellar. Bubbles were coming up but due to well configuation unable to tell which annulus they were coming from. It was either the 20 X 13 3/8" or the 13 3/8 X 9 5/8". Bubbles were coming up in 3 spots. Also on site was Gary Byron, Jesse Bonnice, and Steve Osburne.Workover rig was on site on the 2H to clean out and perf in prep for frac. Need to get containment in shape. |

2489933 | Primary Facility | 2016-05-31 | Administrative/File Review | No Violations Noted | This administration inspection is created to document a spill of production fluid that occurred at the site on 5/11/16. The Department received an e-mail in reference to the spill incident from the PADEP Northeast Regional Office Emergency Response Team on 5/17/16 at 10:33 AM. Matt Shope and Frank Hoehle, DEP O&G WQS, and I responded to the spill incident on 5-17-16 at 13:30. There was no activity or personnel at the site during the inspection. It had been apparent the spill had been remediated. Stone was placed over the original pad material on the pad surface impacted by the production fluid spill. Conductivity of water in the production fluid tanks containment structure near the containment wall and one of the tanks was (9) milliSiemens/cm³. I did not record any elevated soil conductivity readings of concern in the general area of the pad surface where the spill occurred. There is staining/discoloration of water and /or fluid on the side walls of the production fluid tanks and the containment l |
