The Site ID sign and E/S Plan were present at the entrance of the access road. The pit had approximately 5' of freeboard. Drillers log was on-site and being maintained by company man.

1811405 Primary Facility 2009-07-07 Follow-up Inspection No Violations Noted On July 4, 2009, at approximately 9:30 a.m., I received a call from the Company Man at this location. He indicated that an incident had occurred at the site on the evening of July 3rd. According to the Company Man, the crew was in the process of plugging the hole. A plug was set from 6250 to 9000', and 2700 to 4700'. The crew had just set the second plug and reverse circulated the cement through the drill pipe. As the crew started to trip pipe out of the hole, the pipe-rams were mistakenly left shut, and the top of the derek was bent over as a result. One man who was on the derek at the time of the incident was injured with cuts to the face and broken facial bones. No there injuries were reported. A small amount of drill mud (< 1 bbl) was spilled on location. The Company Man indicated a small amount of hydraulic fluid was spilled onto the rig floor and was immediately cleaned up. No other spills were reported.
