2670529 Primary Facility 2017-12-13 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected On 12/13/17 an incident inspection of the Bare Warren was conducted in response to an operator reported spill. The following was observed:

>>The operator reported that a damaged valve on a water truck resulted in a spill of roughly 25 gallons of produced water to the surface of the pad. The spill was reported to have occurred at around 6:30am on 12/13/17.

>>Removal of impacted material was nearly complete at the time of inspection.

>>Two roll off boxes of material were removed. Much of the material was not impacted and was collected due to equipment constraints.

>>While this spill constitutes a violation of 25 Pa. Code 78.54 in that the operator failed to control production fluids and prevent them from reaching the pad the violation is regarded as immediately corrected.

2760398 Primary Facility 2018-08-03 Routine/Complete Inspection No Violations Noted On 8/3/2018 at approximately 1100, I conducted a routine inspection of the Bare Warren Well Site located in Smith Township, Washington County. The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • All wells on site were clearly marked and appeared to be in good condition.
  • Production fluid/condensate tanks were in secondary containment and appeared to be in good condition.
  • GPUs appeared to be in good condition.
  • Site appeared to be stable with greater than 70% vegetation growth.

No violations were found during this inspection.

2788948 Primary Facility 2018-09-18 Routine/Complete Inspection No Violations Noted On 9/18/2018 at approximately 0920, I conducted a routine site inspection of the Bare Warren well site located in Smith Township, Washington County. I met with Nate Smiell (Range Resources) to assess permit termination conditions. The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • All wells were clearly marked and appeared to be in good condition.
  • Production fluid/condensate tanks were within secondary containment and appeared to be in good condition.
  • GPUs were not in secondary containment but appeared to be in good condition. Wells at the site were put into production prior to October 2016.
  • A gravel well pad which was previously occupied by water storage tanks is now being utilized as a staging area by the operator. Written consent of the land use as a staging area would need to be provided by the landowner before the ESCGP could be terminated.

No violations were found during the inspection.

2819356 Primary Facility 2018-12-10 Routine/Complete Inspection No Violations Noted On 12/10/2018 at approximately 11:00, I conducted a site restoration inspection of the Bare Well site located in Smith Township, Washington County. On 9/18/2018, I along with Nate Smiell (Range Resources) assessed site conditions relative to permit termination and it was determined that all requirements pertaining to permanent stabilization and permit termination had been attained. As a result, Range Resources submitted a formal written Notice of Termination to the Department. During the 12/10/2018 inspection, the following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • All wells on site were marked and appeared in good condition.
  • GPUs on site appeared in good condition.
  • The site appeared stable with greater than 70% vegetative cover.
  • Silt fence was left on site by the operator and removed following notification by the Department.

Based on the inspection of the project area, it was determined that all requirements related to permanent stabilization and permit ter

2825669 Primary Facility 2019-01-11 Compliance Evaluation No Violations Noted On 1/11/19 a compliance evaluation inspection of the Bare Site was conducted in response to violations noted on 12/13/17. The following was observed:

>>No residual impact was found in the area of the spill.

>>An accessory pad on the site was handling heavy truck traffic with many parked and idling trucks and trailers. The pad was poorly stabilized with substantial mud and tracking on site. No impact was observed off pad or on nearby roadways. Improvement of pad surface recommended.

2831586 Primary Facility 2019-01-25 Routine/Complete Inspection No Violations Noted This well appears to be be in good condition and well maintained. Well is currently shut-in.
2831721 Primary Facility 2019-01-25 Routine/Complete Inspection No Violations Noted At the time if inspection the Falcon Drilling rig #29 and actively drilling API 125-28637. The drill rig was placed on containment. The Act 9 site ID sign was installed. The PPC plan, ER plan and waste control and disposal plan were on site. The site was covered with approximately 2 inches of snow make the assessment of accelerated erosion or sedimentation difficult. The well is drilled and equipped for production. The Operator representative Lee Mitchell indicated that the well is shut during active drilling operation.
