2805083 Primary Facility 2018-11-19 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a pre-operational inspection. Wash rack is installed with water source and power washer on-site. Construction still occurring on access road and well pad. In general, well pad is about 70% complete. As Photo #5 and #6 show, tank/well pad slopes are still under construction and have not had erosion control matting installed yet. Approximately 80% of site covered in snow. All E&S BMP's appear to be installed as per plan. No violations noted.
2806970 Primary Facility 2018-11-27 Drilling/Alteration No Violations Noted At time of inspection.

On site for a new spud inspection. Met with Tim Magee of United Drilling. United Drilling was on site to drill and run the conductor pipe in the 1H, 3H, 5H and 7H wells. All four wells were drilled to a TD of 94'. The 20" conductor pipe was run in the 1H and 3H wells. They were running the conductor pipe in the 5H when I arrived on site. They plan to run the conductor pipe in the 5H and 7H today and cement all four wells tomorrow. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2844015 Primary Facility 2019-02-25 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Will Dean. PD 181 was on site to drill and run the surface and intermediate casings. There are seven wells to be drilled. The 7H, 5H and 1H wells were drilled with the surface and intermediate casings set and cemented. They were drilling the hole for the intermediate casing of the 3H when I arrived on site. The 2H, 4H and 6H were conductors only. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
