A dozer performing earthwork on the Precision Capital well site burst an o ring allowing approximately 2 3/4 gallons of hydraulic oil to contact the ground. J P Reilly representative Jose Fernandez was performing cleanup and estimated approximately 2-3 tons of material was excavated and stored in a rolloff container destined for a landfill to be determined after testing by Resource. The site was just about cleaned up by the time I arrived and the dozer had been repaired.
2665284	Primary Facility	2017-11-29	Drilling/Alteration	No Violations Noted	Arrived on location at: 13:45. Weather, clear, mild ~ 50 degs. United Drilling onsite setting 20" Conductor and 16" Mouse holes for (4) new wells on this pad, the Precision Capital 1H, 2H, 4H, and 6H. Met with Cabot's WSS onsite Ari Davila. United Drilling is presently rigged up over the 1H well air drilling 24" hole at a depth of ~ 80'. Plan to set 20" conductor for all (4) wells in consolidated rock or ~ 130', which ever comes first. All (4) 20" conductors will be cemented to surface. Cabot plans to be using Halliburton for conductor cementing. - All well permits were valid and present in entry mail box. All was found in good condition at time of inspection. No violations were noted. Left location at: 14:40.

Site has been built to the specifications of the E&S Plan Proposed Conditions 1 Sheet ES-1 dated 10/18/17. No violations noted. Pad 98% complete. All slopes completed with >90% seeded and mulched.
2669961	Primary Facility	2017-12-12	Drilling/Alteration	No Violations Noted	Arrived on location at: 11:30. Weather, overcast, cold ~ 35 degs. United Drilling has setting 20" Conductor and 16" Mouse holes for (4) new wells on this pad, the Precision Capital 1H, 2H, 4H, and 6H. I met with Cabot's WSS onsite Sam Morgan. Sam confirmed that 20" conductor for all (4) wells was set at ~ 145' GL and have been cemented to surface using Halliburton. All wells have had Cellar Tech cellars units set in place. Welders were welding the 20" conductor and 16" mouse hole seams at bottom of cellars at time of inspection. - All well permits were valid and present in entry mail box. All was found in good condition at time of inspection. No violations were noted. Left location at: 12:00.
2691972	Primary Facility	2018-02-05	Drilling/Alteration	No Violations Noted	Arrived on location at: 14:00. Met w/ WSS James Cheeks. H&P #259 Rig is presently over the 1H well, has drilled out 9 5/8" casing shoe and is drilling the 8 3/4" production hole at a depth of ~ 2400'. Drilling on WBM to a projected KOP of 5005'. This Rig has batch drilled the Surface and Intermediate intervals of the 1H, 2H, 6H and 4H wells. All Surface and Intermediate casings have been cemented to surface using Schlumbergers "CemCrete" blend. (See individual insps. for each well for casing depths and cement info). I reviewed, Drilling Logs, Press. Test charts and Cement Job Logs. All were found in order at time of inspection. Took lap around Rig and Rig Floor accompanied by Mr. Cheeks and H&P Drilling Tool Pusher, Fred Jones. All was found in good condition, no violations were noted at time of inspection.Left location at 16:00. (A previous hydraulic oil spill was reported to the Dept. on 2/1/18. 2694180	Primary Facility	2018-02-05	Incident- Response to Accident or Event	No Violations Noted	I arrived at the site at 12:29 in response to a reported spill of (20) gallons of hydraulic fluid to the pad surface. The weather conditions were sunny, cold and breezy with temperatures in the 20's. H&P Flex Rig #259 is the drilling rig at the location. I met with James Cheek, site company man and Fred Jones, Toolpusher, both consultants of Cabot in the company man trailer shortly after I arrived. The current drilling depth is (2,216.78) feet. Mr. Cheeks and Mr. Jones stated the following regarding site activities and the spill. "We are drilling in the vertical section of the 1H well. The intermediate casing was cemented at (2,125)feet on 1/24/18. The intermediate casing has been cemented on the 1H, 2H, 4H and 6H wells. We walked back to the 1H well and resumed drilling with water based mud. Kickoff point is at (5,005) feet. The well will be drilled to total depth at (16,227) feet. We are batch drilling. The spill occurred on 2/1/18 at 19:30. A cap was off and a valve was left open on a hydraulic power unit
2709431	Primary Facility	2018-03-09	Incident- Response to Accident or Event	No Violations Noted	I arrived at the site at 11:34 in response to a spill of an estimated (2) barrels of diesel based drilling mud to containment. H&P Flex Rig #259 is at the site working on the Precision Capital 2H well. I met with Eric Peterson, Drilling Environmental Health and Safety Consultant, Santos Gomez, Drilling HSE manager and Joel Franks, site company man all of Cabot. The weather conditions are cloudy skies, breezy and cold with temperatures in the 30's. Cabot provided me the following information regarding site activities and the spill. "The well was drilled to total depth (TD) at (23,342.23) feet on 3/7/18. We are coming out of the hole at a depth of (8,211.18) feet, (200) feet past the bottom of the curve. We are backreaming the casing and tripping pipe out the well. Production casing may be cemented tomorrow or Sunday. We will move to 6H well next and start drilling from the intermediate casing. The 1H well is drilled to total depth. The spill was discovered on 3/6/18 around 2PM. Oil based mud mixed with diesel
2714057	Primary Facility	2018-03-15	Follow-up Inspection	No Violations Noted	I arrived at the site at 10:39 in response to a report of compromised secondary containment liner integrity to the containment used for the mud pumps. The weather conditions are sunny skies with a light to moderate breeze and temperatures in the 30's. Brody Webster, Senior Environmental Health and Safety Specialist of Cabot contacted me at 8:15 AM on 3/15/18 to indicate that Cabot has pulled the rig mats in the area where the diesel based drilling mud spill occurred which was discovered on 3/6/18, to evaluate the containment. Mr. Webster informed me during the phone conversation that a hole was discovered in the duck pond (equipment) containment for the mud pump house. The fluid was spilled out of one of the mud pumps located inside the house to the duck pond containment, the rig mats below and the felt below the rig mats. 2727633	Primary Facility	2018-05-03	Drilling/Alteration	No Violations Noted	Arrived on location at 14:30. Met with WSS Joel Franks. H&P X36 Rig presently RU over the 4H, drilling prod. hole at a depth of 4,850' to a projected KOP of ~ 5,835'. Rig has recently skid off the 6H well after completing drilling, casing and cementing the 5 1/2" Prod. csg., set at a depth of 22,777' on 4/30/18. (See insp. report for the 6H well for casing and cementing details). - I reviewed 6H wells Prod. csg cement job report and BOP, Intermediate csg. test and FIT test charts for the 4H well. All were found in good order. - The 4H well will be the last well drilled on this location, at this time. A total of (4) wells, the 1H, 2H, 6H & 4H have been drilled on pad. Overall Rig and location were found in good condition at time of inspection, no violations were noted at time of inspection. Left location at 15:30.
2728223	Primary Facility	2018-05-09	Complaint Inspection	No Violations Noted	Arrived on location at 09:00. Onsite in response to report from concerned citizen that "diesel fuel is being injected into ground". (Complaint ID #334121). - Upon arrival I met with Cabot Co. Man, Joel Franks. The H&P #259 Rig was drilling the 8 1/2" lateral section of the 4H well on Diesel Oil Base Mud "DOBM" at a depth of 13,775'. The Surface and Intermediate intervals have been drilled, cased and cemented, drilling on Fresh Water Base Mud "FWBM". (Intermediate csg. set at 2100'). The remainder of vertical section of well was drilled to Kick off point "KOP" ~ 5800', also drilling on FWBM. Operator then loaded hole with DOBM to build curve and drill lateral to TD. This is normal, standard drilling practice, in compliance with Regulations. - While drilling, drilling fluids are "Circulated", down drill pipe and up annulus to lubricate bit and bring cuttings to surface. At no time is DOBM or any drilling fluid "Injected" into ground. - I took a lap around Rig and location accompanied by Mr. Franks. All Diesel
2750400	Primary Facility	2018-07-05	Complaint Inspection	No Violations Noted	Arrived on location at 14:15. On location in response to Complaint #335350 received by The Dept. on 7/3/18. Complaint included a video, recorded at night, which was labeled "Silica in the air". Upon arrival at site I met with Cabot O&G Reps., Completion Consultant Jody Martin, E.H.S. Reps. Ken Wilson and Wayne Wilridge and Keane Groups Site Safety Rep. Peter Barbano. - Completions Operations have been underway on location since ~ 6/25/18. All (4) wells on pad, the 1H, 2H, 4H & 6H wells are being Stimulated. Currently, stages that have been completed were: 1H: 3 of 34, 2H: 18 of 88, 4H: 13 of 83 and 6H: 14 of 79. The average formation breakdown pressure has been ~ 7500 psi. Mr. Martin stated that operations have gone relatively well with no major issues encountered. - I discussed the complaint received by The Dept. and we viewed the video received with complaint. On 8/10/18, COG contacted the Department to report the release of produced fluid on their Precision Capital Well Pad. The release occurred from a transfer operation from a Susquehanna Gas Fields Services truck to the tank farm on the well pad. A driver error caused over-pressurization of the load which caused 20 to 25 gallons of recycled produced water to be released to the pad surface. The spill occurred around 1300 hrs with DEP notification having occurred around 1430 hrs. Present during today's inspection was Ken Simmons, COG EH&S Supervisor. Visibly impacted soils were excavated and placed directly into a roll-off container. The excavation measured approximately 30' by 10' by 1' deep. Resource Environmental was contracted to oversee remediation efforts. All post-remediation readings were less than 300 uS/cm and the excavation was subsequently back filled. The weather conditions are mostly sunny skies, very warm and humid with temperatures in the mid to upper 80's. Steve Watson, Environmental Health and Safety Specialist of Cabot Oil and Gas was at the site during the inspection. Brody Webster, Safety and Environmental Manager of Cabot contacted me on 8/24/18 at 14:16 and Mr. Watson contacted me on 8/27/18 at 7:56 to inform me that Cabot was planning on removing rig mats, secondary containment liner and felt in the general area of the pad surface where an estimated (12-13) barrels of diesel based drilling mud was spilled to containment during the week of 3/5/18, on 8/27/18 or 8/28/18 in order to evaluate the pad surface for a potential impact due to compromised liner integrity. 2819702	Primary Facility	2019-04-01	Routine/Complete Inspection	No Violations Noted	I performed a RTNC inspection of the Precision Capital wellsite. The weather was sunny and about 34 degrees. The wind was blowing slightly. There was a Rain For Rent frac tank sitting onsite. It appeared to be empty. The access road was solid and stable with an infiltration berm BMP situated near the entrance. The pad surface was solid as well. The cut and fill slopes were well vegetated, and the fill slope had a large infiltration berm BMP of stacked filter sock along the fill slope. The wells were all properly labeled. There were no violations noted at the time of inspection.
