2631198 Primary Facility 2017-08-30 Routine/Partial Inspection No Violations Noted At time of inspection.

Conducted a routine inspection for a possible migration investigation. I was accompanied by Trudy Graby (Oil and Gas Inspector Supervisor). We met with Steve Brominski, Ken Wilson, and Steve Watson of Cabot Oil and Gas. The annuli were disconnected in the cellar so we were unable to check for gas shows, but we noticed latex gloves on the annuli and the gloves on the (13x9) annulus of the 10H, 11H, 12H, and 14H appeared to be inflated. Steve Brominski said Cabot would have the annuli plumbed to the surface for inspection access. The department will return at 1:00 p.m. on 8/31/17 to check for gas show and take samples if gas is detected.

2634165 Primary Facility 2017-08-31 Complaint Inspection Violation(s) Noted Upon arrival to site, Cabot has had all (6) wells annuli, the (20¿ x 13 3/8¿), (13 3/8¿ x 9 5/8¿) and (9 5/8¿ x 5 ½¿) separated and plumbed to surface. All annular vents have been open and venting separately for approx. 4 hrs prior to screening.

All (6) wells, the Foltz, J. 9H, 10H, 11H, 12H, 13H and 14H were recently drilled to TD and have been toe prepped in preparation for well stimulation operations. All wells were shut in at time of inspection.

Personnel Present: Cabot O&G Reps: Steve Watson, Jerry Dugas and Mike Bail. Selma Associates: Matt and Bret. DEP Reps: Eric Rooney and Myself.

DEP gas screenings were performed using a ALTAIR 5X gas meter calibrated on 8/30/2017. All samples taken by DEP were also taken by Selman Associates on Cabots behalf.

2643380 Primary Facility 2017-09-06 Routine/Complete Inspection No Violations Noted Followup visit from previous week to determine if any additional equipment was mobilized onto site for frac ops. At time of inspection, no additional equipment was mobilized to location for frac ops. Wireline was on location, however was sitting idle and no activity was taking place at time of inspection.
2657055 Primary Facility 2017-10-19 Routine/Complete Inspection No Violations Noted I arrived at the site at 11:35. The weather conditions were sunny skies with temperatures in the low to mid 60's, with light winds. I met with Travis Routan, driller of Stone Well Service. Mr. Routan provided the following information in reference to site activities. "We have been squeezing cement in the 13H and 10H wells. Just completed the 13H. The cement is coming up the back side of the production casing. The intent of squeezing the cement is to keep the gas in the production casing. The rig has been laid down today and will be going to the Cabot Howell Pad to perform a similar task. The Erick Flowback Services gas buster tank on containment is there to contain a kick of gas if it occurs. The blue tank is cement and water that comes out of the well. Sawdust is in the red tank. It is used for the wet cement solidification and mixed in the blue tank. Green frac tanks will be used during the frac." I left the site at 12:43.
2670908 Primary Facility 2017-12-04 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 12:02 in response to a reported spill of (12) gallons of diesel fuel that occurred on 12/2/17. The weather conditions are partly cloudy skies with temperatures around 40 degrees. I met with Wayne Wilridge, Safety and Regulatory Compliance and Ken Wilson, EHS Completions Foreman, both consultants of Cabot Oil and Gas. Mr. Wilson and Mr. Wilridge informed me of the following details associated with the spill. "We were first notified of the spill at 1:40 PM on 12/2/17. The spill occurred as a result of a fuel cap not secured on the tank in the back of a pickup truck. The truck picked up fuel in a (1,000) gallon in capacity above ground fuel cell on the Foltz pad where Keane is conducting the frac. The truck was headed to the Arnone to fuel generators and heaters on the tank pad section of the site. Diesel fuel overflowed the tank onto the truck bed and flowed out of the corner of the tailgate to the Foltz pad access road near the pad entrance, on the entire length of the Foltz pad acces
2677341 Primary Facility 2017-12-27 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected At 2:30 pm on December 26, 2017 during flowback operations on the Foltz J Well Site, Baker had a saddle blow out on the water manifold due to a valve not closing all the way on the tank battery. The tank battery is not currently operational and is in the process of being disassembled. The company is now working out of water trucks. About 2 to 2.5 bbls of flowback water hit the ground.

The release was immediately vacuumed up with a vac truck on site. Approximately 12 yards of impacted material was dug up and was still on site in a lined rolloff container awaiting disposal.

Resource Environmental performed screening on December 26 and 27, 2017 during excavating. Screening was measured below 300 uS during excavation. According to Ken Wilson, Env'l Health and Safety contractor, the rolloff container will remain on site pending a decision on where it is to be sent. Resource representatives had recently left and were not on site at the time of inspection.

2677556 Primary Facility 2017-12-20 Routine/Complete Inspection No Violations Noted Arrived onsite at 13:45. The stimulation operations of all (6) wells on pad has been completed. (2) rigs were onsite in process of performing plug DO operations. Rigs were presently rigged up completing DO on the 10H and 13H wells. Upon arrival I met with Cabot WSS, Steve Brominski and Cabots HSE Rep. onsite.

Mr. Brominski informed that DO operations have been completed on the 9H, 11H, 12H and 14H wells. Rig was presently finishing up on the 13H and Rig on the 10H was approx. half way through DO. He anticipates being completed with DO operations sometime tomorrow. I asked whether any issues had been encountered during stimulation or DO ops. Specifically in the 10H and 13H wells due to the prod. casing patches installed in these wells during remedial work performed prior to stimulations. No issues had been encountered to this point.
