The water was used during pressure testing prior to hydraulic fracturing operations when the sightglass broke on vacuum unit. Contaminated material was immediately excavated and disposed of as the necessary equipment was already on site.

2566042 Primary Facility 2017-02-22 Routine/Complete Inspection No Violations Noted The Department (Eric Rooney and I) stopped at the well site after finding that the road seemed to have been collecting sediment from vehicles leaving the site earlier in the day as we were en route to a meeting. At the time of our inspection, it was noted that it appeared as though the access road had recently been maintained. Additionally, it appeared as though sediment was no longer leaving the site in the amount it was prior.
2568357 Primary Facility 2017-02-24 Follow-up Inspection No Violations Noted The Department returned to the site to conduct a Follow-Up Inspection concerning the sediment being deposited on State Route 29. Ken Wilson (EHS Foreman -Cabot Completions) was on site and walked along the access road and SR 29 with me. I also met with Larry Fulmer (Frac Superintendent). We walked along the road and found that the sediment mentioned in the Department's 2/22/2017 inspection is still present. Mr. Wilson explained that they have added rock to an area adjacent to the access road and that they have been adding stone to the access road which has built it up.

While on site I explained that any sediment that originated from their site should be collected and returned to within their limits of disturbance (LOD) and stabilized. I requested that they come up with a plan to remove the sediment in a timely and safe manner. Mr. Wilson also explained that they will be relocating a small pile of sediment adjacent to their access road, near the base of the road, and stabilizing it.

2570106 Primary Facility 2017-03-08 Routine/Complete Inspection No Violations Noted I conducted a Follow-Up Inspection in response to Steve Watson's (Cabot) email dated 2/27/17 concerning the clean up of sediment accumulations along SR 29. Photos attached to the email were dated 2/24/17.
2659495 Primary Facility 2017-11-13 Routine/Complete Inspection No Violations Noted Arrived on location at: 10:00. Weather: Winter mix, Cold, ~ 35 degs. A total of (9) wells have been drilled on this pad, the Kropa, T. 1H, 3H, 4H, 5H, 6H, 7H, 8H, 9H and 10H. All wells were in production at time of inspection. All wells (13x9) and (9x5) annuli were open and venting, plumbed to a common vent tank. The 5H, 6H, 7H, 8H, 9H and 10H wells (20x13) annuli were also open and venting, plumbed to a common vent tank. A Gas check performed at the common annular vent tank showed 100% LEL, 100% CH4. -All wells Prod. casing, Static and Differential pressures were being monitored by wireless sending units to onsite Total Flow Control Unit "TFCU". (See individual well insp. reports for pressures.) - TFCU readings were taken off control panel at ~ 10:45. (SP= static press., CP= prod. casing press., DP= differential press.) - 6H: SP= 541 psi, CP= 601 psi., DP= 109 psi. - Overall, wells and pad were found in good condition. No obvious concerns noted at time of inspection. No violations noted. Left location at 11
2659922 Primary Facility 2017-11-13 Routine/Complete Inspection No Violations Noted Ken Kennedy was on site during the inspection. There was no activity occurring at the site at the time of inspection. There were no violations noted.
2834532 Primary Facility 2019-02-01 Routine/Complete Inspection No Violations Noted I arrived on site at 1449 to conduct a site restoration inspection in response to a post-drilling site restoration report received from the Operator. The weather was ~38 degrees F and cloudy with moderate freezing rain. There was no activity onsite at the time of inspection. The pad surface was plowed but extremely icy.

Vegetative stabilization requirements were unable to be assessed due to snow cover. Another inspection will have to be conducted when the snow melts in order to determine whether the site meets the post-drilling restoration requirements.
