2452456 Primary Facility 2016-02-23 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:50 p.m., United Drilling was on site; I met with company man Mark Talarico and Selman representative Brett Wambold. There were two rigs drilling the holes for the conductor pipe, one rig was drilling the 4H which was at 81' and the other one was drilling the 3H which was at 23'. Mark and Brett said they should be cementing the conductor pipes on the 1H, 3H and 4H on 2/26/16. I took a walk around the location and everything looked OK.

I left the site at 13:30 p.m.

2463554 Primary Facility 2016-03-08 Incident- Response to Accident or Event No Violations Noted Cabot removed an approximate 12-15 cubic yards of material at the time the Department arrived. They stated that the machine sat from the time of the release until the morning of 3/8/16, when they were able to fix and then move it. It was reported that the bulldozer’s hose clamp broke and the hose disconnected from the reservoir. The tank was described as having a 15.8 gallon capacity. Cabot Oil & Gas presumes that they lost almost the entire volume of the tank, which is how they estimated the 15 gallons. The Department observed the excavation and noted that it appeared as though there was some remaining product floating on the water in the excavation. 2474745 Primary Facility 2016-04-14 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:00 a.m., Precision Rig 548 was on site rigged up over the 4H and was drilling the hole for the surface casing. I met with company man Perk McDowell, I took a walk around the location and everything looked OK.

Depth at time of inspection was 741'. I left the site at 12:20 p.m.

2482222 Primary Facility 2016-05-09 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 11:05 a.m., Precision Rig 548 was on site and was drilling the hole for the production casing. I met with company man Marcus Cormier, we took a walk around the location and everything looked OK.

Depth at time of inspection was 4959'. Projected TD is 15800' I left the site at 12:15 p.m.

2488376 Primary Facility 2016-05-16 Incident- Response to Accident or Event No Violations Noted On 5/12/2016, David Schoonover, PADEP OGI, received a text message on his work iPhone on 5/12/16 from the iPhone number of (412) 980-8973 regarding a hydraulic fluid spill. The text message stated the following. "Hey, we had a hydraulic line fail on the loader. 2 gallons on top of primary contaiment containment and mats. I'll have a report in the morning. I wasn't sure if I needed to let you know or not." Mr. Schoonover responded back via a text message to the unknown individual who sent the text inquiring if it was the J. Housenick Pad 1 that he/she was referring to where the spill occurred. The individual responded "yes" to Mr. Schoonover's inquiry.

Precision Drilling Rig #548 is at the location. The most recent drilling resumption notice submitted to the Department from Cabot was on 4/12/16. The notice indicated that Cabot would resume drilling after 30 days or more of notification on the J Housenick 3H well on 4/1/4/16 at 4:00 PM. 2657061 Primary Facility 2017-11-06 Routine/Complete Inspection No Violations Noted While out inspecting the Rigsby pipeline, I did a quick inspection on the Housenick well site. The site and access road were solid and stable with no erosional issues noted. The vegetation was healthy and sufficient.

2725857 Primary Facility 2018-05-02 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Housenick well site. The wells associated with this site are as follows: 115-22096, 115-22100, 115-22101. The wells were properly labeled.

The access road and pad surfaces were solid and stable.

The cut and fill slopes were well vegetated and were stable. There were no erosional issues apparent.

There were no violations.
