2500375 Primary Facility 2016-07-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m., the cellar was about half full of water with no bubbling noticed, the (20x13) vent pipe was open and venting into the atmosphere with no gas detected, the (13x9) vent pipe was open and venting into the atmosphere with no gas detected, the (9x5) vent pipe was open and venting into the atmosphere with gas detected at 100%LEL and 100% methane with no measurable flow. There was 880psi on the production casing and 574psi on the production tubing. I took a walk around the location and everything looked OK.

I left the site at 13:30p.m.

2586951 Primary Facility 2017-04-25 Routine/Complete Inspection No Violations Noted The site was stable with no erosional issues currently. Vegetation was healthy and greening up nicely. There was no bubbling in the well cellars and the access road was stable. The last company inspection was 4-20-17
