2316021 Primary Facility 2014-10-24 Routine/Complete Inspection No Violations Noted The Department conducted an inspection of the Cabot Biniewicz, S. Pad on 10/24/14 in response to a nearby water supply complaint. There are 6 wells drilled and in production on this pad. They are: Biniewicz 1H (115-21313), 2H (115-21314), 3H (115-21315), 4H (115-21316), 5H (115-21317), and the 6H (115-21318). The inspection revealed that the pad was constructed per the plan. There has been no drilling activity on this pad for the last 6 months. No violations were observed at the pad site.

2513811 Primary Facility 2016-08-25 Routine/Complete Inspection No Violations Noted On 8/25/16, the Department conducted an inspection of the COG Biniewicz, S. Pad located in Gibson Township, Susquehanna County. Last Cabot E&S inspection documented at site as having been conducted on 8/22/16. The entrance to the access road is also the shared driveway and parking area for a house located at the entrance. The access road is paved for about 50’. All six wells have been drilled, stimulated, and appear to be in production. The well pad berms have been eliminated. The tank pad has been reclaimed with what appears to be the tank pad modified material for the pad having been used to create an access road presumably for the landowners use to access his property from the well pad access road. The new owner access road cuts across the old tank pad location and proceeds to the wood line across the diversion swale. A cross culvert pipe has been placed in this swale. The culvert pipe is elevated approximately 1’ from the contour of the diversion swale. Recent heavy rain events suggest this cha

2542189 Primary Facility 2016-12-07 Routine/Complete Inspection No Violations Noted The Department was on site, to conduct a Routine Complete Inspection, in response to the submittal of a Restoration Report. The E&S Inspection Log indicated that Stephen P. Opteka conducted the most recent inspection on 12/1/16 and it was a Post Rain Event Inspection.

On site it was noted that what appeared to be new filter sock was situated along the access road and well site and it appeared to be in place for the pipeline. The PCSM/ Site Restoration Plan 1 Map noted that the proposed earthen level spreader has been substitutued for an existing subsurface level spreader.

The tank pad has been removed and is now vegetated.

2552333 Primary Facility 2017-01-12 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:35a.m., the cellar was about three quarters full of water and ice with no bubbling noticed. The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 604psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 12:15p.m.

2639965 Primary Facility 2017-09-26 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was not producing. There was containment at the 5H and appeared tubing was recently put in the wells. All the annulus vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2655031 Primary Facility 2017-10-26 Site Restoration No Violations Noted This inspection was conducted to confirm Cabot's assessment that the project meets the minimum requirements for permanent stabilization. The Departments determination is that the site meets the requirements for permit termination.

I met with company man James Restly. Sun Well Service Rig 2 was rigged up on the 1H well. The well was communicated with while Cabot was stimulating the wells on the Jeffers Farms Pad 5. While flushing out debris, the tools got stuck in the hole at 7414', they cut off the 2 3/8 pipe at 7104'. They were running in the hole with wash over tools when I arrived on location. There are six wells on site and the 1H is the only one that was affected. I took a walk around the location and there were no obvious concerns noticed. There were no violation noted.

2745573 Primary Facility 2018-06-26 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2844923 Primary Facility 2019-02-26 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was ice 1.5' below grade in the cellar and I was unable to check for gas. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
